摘要:
目的 探讨特布他林联合布地奈德雾化吸入治疗儿童感染后闭塞性细支气管炎的临床效果。方法 选取2014年8月至2016年8月随州市妇幼保健院收治的感染后闭塞性细支气管炎患儿60例,根据治疗方法分为观察组和对照组,每组30例。2组患儿均给予吸氧、抗感染等常规治疗,在常规治疗基础上,对照组患儿给予布地奈德雾化吸入治疗,观察组患儿给予特布他林联合布地奈德雾化吸入治疗,比较2组患儿临床症状恢复情况、临床疗效及血清炎性因子水平。结果 观察组患儿咳嗽、气促、喘憋及肺部啰音消失时间显著短于对照组(P<0.05)。治疗前2组患儿血清白细胞介素-4(IL-4)和γ-干扰素(IFN-γ)水平比较差异均无统计学意义(P>0.05);治疗后2组患儿血清IL-4水平显著高于治疗前,IFN-γ水平显著低于治疗前(P<0.05);治疗后观察组患儿IL-4水平显著高于对照组(P<0.05),IFN-γ水平显著低于对照组(P<0.05)。观察组和对照组患儿治疗总有效率分别为93.33%(28/30)、76.67%(23/30),观察组患儿治疗总有效率显著高于对照组(χ2=7.231,P<0.05)。结论 特布他林联合布地奈德雾化吸入治疗可以有效改善感染后闭塞性细支气管炎的临床症状,减轻炎症反应,提高治疗效果。
Abstract:
Objective To investigate the effect of terbutaline combined with budesonide inhalation in the treatment of children with bronchiolitis obliterans after infection.Methods Sixty children with bronchiolitis obliterans after infection were selected from August 2014 to August 2016 in Suizhou Maternal and Child Health Hospital.The patients were divided into observation group and control group according to the treatment methods,thirty cases in each group.The children in the two groups were treated with oxygen inhalation,anti-infection and other conventional treatment.The children in the control group were treated with budesonide inhalation on the basis of routine treatment.The children in the observation group were treated with terbutaline combined with budesonide inhalation on the basis of routine treatment.The clinical symptoms recovery,clinical effect and serum inflammatory factors levels were compared between the two groups.Results The extinction time of cough,shortness of breath,asthmatic suffocation and pulmonary rale in the observation group was significantly shorter than that in the control group (P<0.05).There was no significant difference in serum interleukin-4(IL-4) and interferon-γ(IFN-γ) levels between the two groups before treatment (P>0.05).The serum IL-4 level after treatment was significantly higher than that before treatment,and the serum IFN-γ level after treatment was significantly lower than that before treatment in the two groups (P<0.05).The serum IL-4 level in the observation group was significantly higher than that in the control group (P<0.05),and the serum IFN-γ level in the observation group was significantly lower than that in the control group after treatment (P<0.05).The total effective rate in the observation group and the control group was 93.33% (28/30) and 76.67% (23/30) respectively,the total effective rate in the observation group was significantly higher than that in the control group (χ2=7.231,P<0.05).Conclusion Terbutaline combined with budesonide inhalation therapy can effectively improve the clinical symptoms,reduce the inflammatory response and improve the therapeutic effect in children with bronchiolitis obliterans after infection.
