摘要:
目的 探讨沙美特罗替卡松粉吸入剂联合氨茶碱注射液治疗老年支气管哮喘的临床效果。方法 选择2015年1月至2018年12月空军军医大学第一附属医院收治的186例老年支气管哮喘患者为研究对象,根据治疗方法将患者分为观察组和对照组,每组93例。2组患者均给予解痉、祛痰、止咳、抗感染等常规治疗,在常规治疗基础上,对照组患者给予沙美特罗替卡松粉吸入剂550 μg,经鼻吸入,每日2次,治疗3个月;观察组患者在对照组治疗基础上给予氨茶碱注射液0.25 g,加入50 g·L-1葡萄糖注射液250 mL,静脉滴注,每日1次,治疗3个月。观察2组患者咳嗽、喘憋、哮鸣音、湿啰音等消失时间;对2组患者治疗前后肺功能、血清神经生长因子(NGF)和白细胞介素-6(IL-6)水平、哮喘生活质量问卷(AQLQ)评分进行比较;治疗后评估2组患者的临床治疗效果,并观察2组患者不良反应发生情况。结果 观察组患者咳嗽、喘憋、哮鸣音、湿啰音消失时间均短于对照组(P<0.05)。治疗前2组患者呼气流量峰值(PEF)、用力肺活量(FVC)、第1秒用力呼气量(FEV1)比较差异无统计学意义(P>0.05),2组患者治疗后PEF、FEV1、FVC高于治疗前(P<0.05);治疗后,观察组患者PEF、FEV1、FVC高于对照组(P<0.05)。治疗前2组患者血清NGF、IL-6水平比较差异无统计学意义(P>0.05),2组患者治疗后血清NGF、IL-6水平低于治疗前(P<0.05);治疗后,观察组患者血清NGF、IL-6水平低于对照组(P<0.05)。治疗前2组患者AQLQ评分比较差异无统计学意义(P>0.05),2组患者治疗后AQLQ评分高于治疗前(P<0.05);治疗后,观察组患者AQLQ评分高于对照组(P<0.05)。观察组和对照组患者治疗总有效率分别为96.77%(90/93)、83.87%(78/93),观察组患者治疗总有效率高于对照组(χ2=8.857,P<0.05)。观察组和对照组患者不良反应发生率分别为12.90%(12/93)、8.60%(8/93),2组患者不良反应发生率比较差异无统计学意义(χ2=0.896,P>0.05)。结论 沙美特罗替卡松粉吸入剂联合氨茶碱注射液可有效改善老年支气管哮喘患者的临床症状,抑制炎症反应,提高患者肺功能和生活质量。
Abstract:
Objective To investigate the clinical effect of salmeterol xinafoate and fluticasone propionate powder for inhalation combined with aminophylline injection in the treatment of elderly patients with bronchial asthma.Methods A total of 186 elderly patients with bronchial asthma admitted to the First Affiliated Hospital of Air Force Medical University from January 2015 to December 2018 were selected as the study objects,and the patients were divided into observation group and control group according to the treatment methods,with 93 cases in each group.The patients in the two groups were given routine treatment such as spasmolysis,expelling phlegm,cough relieving and anti-infection.On the basis of routine treatment,the patients in the control group were given 550 μg salmeterol xinafoate and fluticasone propionate powder for inhalation through intranasal inhalation,twice a day for 3 months.On the basis of the treatment in the control group,the patients in the observation group were given 0.25 g aminophylline injection and 250 mL glucose injection (50 g·L-1) by intravenous drip,once a day for 3 months.The disappearance time of cough,gasping,wheezing sound and moist rale were observed.The lung function,serum nerve growth factor (NGF) and interleukin-6 (IL-6) levels,asthma quality of life questionnaire (AQLQ) score of the patients were compared between the two groups before and after treatment.The clinical effects of the patients in the two groups were evaluated after treatment,and the adverse reactions were observed.Results The disappearance time of cough,gasping,wheezing sound and moist rale of the patients in the observation group was shorter than that in the control group (P<0.05).There was no significant difference in peak expiratory flow (PEF),forced vital capacity (FVC) and forced expiratory volume in one second (FEV1 ) between the two groups before treatment (P>0.05).The PEF,FEV1  and FVC of the patients after treatment were higher than those before treatment in the two groups (P<0.05).The PEF,FEV1  and FVC of the patients in the observation group were higher than those in the control group after treatment (P<0.05).There was no significant difference in serum NGF and IL-6 levels between the two groups before treatment (P>0.05).The levels of serum NGF and IL-6 of the patients after treatment were lower than those before treatment in the two groups (P<0.05).The levels of serum NGF and IL-6 in the observation group were lower than those in the control group after treatment (P<0.05).There was no significant difference in the AQLQ score of the patients between the two groups before treatment (P>0.05).The AQLQ score of the patients in the two groups after treatment was higher than that before treatment (P<0.05).The AQLQ score in the observation group was higher than that in the control group after treatment (P<0.05).The total effective rate in the observation group and the control group was 96.77% (90/93) and 83.87% (78/93),respectively.The total effective rate in the observation group was higher than that in the control group (χ2 =8.857,P<0.05).The incidence of adverse reactions in the observation group and the control group was 12.90% (12/93) and 8.60% (8/93),respectively.There was no significant difference in the incidence of adverse reactions between the two groups (χ2 =0.896,P>0.05).Conclusion Salmeterol xinafoate and fluticasone propionate powder for inhalation combined with aminophylline injection in the treatment of elderly patients with bronchial asthma can effectively improve the clinical symptoms,inhibit the inflammatory response,and improve the lung function and quality of life.
