摘要:
目的 探讨中性粒细胞淋巴细胞比(NLR)、环氧化酶-2(COX-2)、核转录因子-κB(NF-κB)在食管鳞状细胞癌中的表达及相互之间的关系。方法 选取新乡医学院第一附属医院经病理诊断为食管鳞状细胞癌的患者60例(鳞癌组),另选择同期健康体检者42例(对照组),分别检测2组受试者外周血常规并计算NLR,比较2组受试者NLR的差异,并进一步分析NLR与食管鳞状细胞癌病理特点的关系。采用免疫组织化学方法检测食管鳞状细胞癌患者癌组织及癌旁组织中COX-2、NF-κB的表达。鳞癌组患者以NLR中位值3.0为节点分为低NLR组(NLR<3.0)和高NLR组(NLR≥3.0),分析2组患者癌组织中COX-2、NF-κB表达的差异。采用Spearman等级相关分析COX-2与NF-κB表达的相关性。结果 鳞癌组和对照组受试者NLR分别为1.79±0.77、3.75±2.22,鳞癌组受试者NLR显著高于对照组(P<0.05)。NLR与患者的性别、年龄、肿瘤大小、肿瘤位置无明显相关性(P>0.05),与浸润深度、淋巴结转移、远处转移、病理分级、临床分期有相关性(P<0.05)。COX-2和NF-κB在食管鳞状细胞癌组织中的阳性表达率分别为81.7%(49/60)和56.7%(34/60),在癌旁组织中的阳性表达率分别为58.9%(35/60)和36.7%(22/60),COX-2和NF-κB在食管鳞状细胞癌组织中的阳性表达率均显著高于癌旁组织(P<0.05)。COX-2表达与肿瘤大小及浸润深度相关(P<0.05),与年龄、肿瘤部位、淋巴结转移、远处转移及病理分级无关(P>0.05)。NF-κB表达与肿瘤浸润深度、淋巴结转移、远处转移有关(P<0.05),与年龄、肿瘤大小、肿瘤部位、病理分级无关(P>0.05)。COX-2和NF-κB在食管鳞状细胞癌组织中表达呈正相关(P<0.05)。高NLR组患者癌组织中COX-2和NF-κB阳性表达率分别为94.1%(32/34)和73.5%(25/34),显著高于低NLR组的65.4%(17/26)和34.6%(9/26)(P<0.05)。结论 NLR可能是评价食管鳞状细胞癌恶性生物学行为的指标;COX-2、NF-κB可能通过促进慢性炎症的过程参与食管鳞状细胞癌的发生、发展。
Abstract:
Objective To observe the expression of neutrophil to lymphcyte ratio(NLR),cyclooxygenase-2(COX-2)and nuclear factor-kappa B(NF-κB) in esophageal squamous carcinoma,and the relationship between each other.Methods Sixty esophageal squamous cell carcinoma patients who were diagnosed by pathology in the First Affiliated Hospital of Xinxiang Medical University were selected as squamous cell carcinoma group;another forty-two healthy subjects were selected as control group at the same period.The NLR was caculated according to the results of blood routine test of all subjects,and then the difference of NLR was compared between the two groups.The relationship between NLR and pathological features of esophageal squamous cell carcinoma was analysed furtherly.Immunohistochemistry was used to detect the expression of COX-2,NF-κB in cacinoma and para-carcinoma tissue of esophageal squamous cell carcinoma patients.The patients in squamous cell carcinoma group were furtherly divided into low NLR group(NLR<3.0)and high NLR group according to the median value(3.0) of the NLR and the difference of COX-2 and NF-κB expression was analysed between the two groups.The correlation between the COX-2 and NF-κB expression was analyzed by Spearman rank correlation analysis.Results The NLR level of subjects in squamous cell carcinoma group and control group was 1.79±0.77,3.75±2.22 respectively;the level of NLR in squamous cell carcinoma group was significanlty higher than that in the control group(P<0.05).There was no obvious correlation between the NLR and patients′ age,gender,tumor size,tumor location (P>0.05);but the NLR closely related to the depth of invasion,lymph node metastasis,distant metastasis,pathologic grade and clinical stage(P<0.05).The positive expression rate of COX-2 and NF-κB in esophageal squamous cell carcinoma tissue was 81.7%(49/60)and 56.7%(34/60),in para-carcinoma tissues was 58.9%(35/60)and 36.7%;the positive expression rate of COX-2,NF-κB in esophageal squamous cell carcinoma tissue was significantly higher than that in para-carcinoma tissue(P<0.05).The expression of COX-2 was related to the tumor size,depth of invasion(P<0.05),while was not related to the age,tumor location,lymph node metastasis,distant metastasis and pathologic grade(P>0.05).The expression of NF-κB was related to the depth of invasion,lymph node metastasis,distant metastasis(P<0.05),while was not related to the age,tumor size,tumor location and pathologic grade(P>0.05).The expression COX-2 in esophageal squamous carcinoma tissue was positively correlated with the expression of NF-κB (P<0.05).The positive expression rate of COX-2 and NF-κB in carcinoma tissue was 94.1%(32/34)and 73.5%(25/34)in high NLR group and 65.4%(17/26),34.6%(9/26) in low NLR group;the positive expression rate of COX-2 and NF-κB in high NLR group was significantly higher than that in low NLR group(P<0.05).Conclusion NLR may be an indicator for evaluating the malignant biological behavior of esophageal squamous cell carcinoma.COX-2,NF-κB may be involved in the development of esophageal squamous cell carcinoma by promoting the process of chronic inflammation.
