摘要:
目的　探讨股骨近端防旋髓内钉（PFNA）内固定治疗股骨粗隆间骨折的临床效果。方法　选择2011年5月至2014年5月宜宾市第二人民医院收治的股骨粗隆间骨折患者80例，根据手术方式分为观察组和对照组，每组40例。对照组患者给予股骨近端解剖型锁定钢板治疗，观察组患者给予PFNA内固定治疗，对2组患者的手术情况、术后并发症及临床疗效进行比较。结果　观察组患者手术切口长度、手术时间、术后下床活动时间、住院时间、骨折愈合时间显著短于对照组（P<0.05），观察组患者术中出血量和术后引流量显著少于对照组（P<0.05）。观察组和对照组患者治疗优良率分别为95.0%（38/40）、90.0%（36/40），2组患者临床疗效比较差异无统计学意义（χ2=0.720，P>0.05）。观察组和对照组患者术后并发症发生率分别为5.0%（2/40）、22.5%（9/40），观察组患者术后并发症发生率显著低于对照组（χ2=5.165，P<0.05）。结论　PNFA治疗股骨粗隆间骨折疗效确切，且具有创伤小、手术时间短、术中出血少、住院时间短、固定牢靠、骨折愈合快及并发症少等优点。
Abstract:
Objective　To investigate clinical effect of proximal femoral nail anti-rotation （PFNA） in the treatment of intertrochanteric fracture.Methods　A total of 80 patients with intertrochanteric fracture were selected from May 2011 to May 2014 in the Second People′s Hospital of Yibin City.The patients were divided into observation group and control group according to the surgical methods，40 cases in each group.The patients in the control group were treated with proximal femoral anatomic locking plate，and the patients in the observation group were treated with PFNA.The operation status，postoperative complications and clinical effect were compared between the two groups.Results　The length of operative incision，operation time，postoperative ambulation time，hospitalization time and the fracture healing time in the observation group were significantly shorter than those in the control group （P<0.05）.The intraoperative blood loss and postoperative drainage volume in the observation group were significantly less than those in the control group （P<0.05）.The fineness rate in the observation group and the control group was 95.0% （38/40） and 90.0% （36/40），respectively；there was no significant difference in clinical effect between the two groups （χ2=0.720，P>0.05）.The incidence of postoperative complications in the observation group and the control group was 5.0% （2/40） and 22.5% （9/40），respectively；the incidence of complications in the observation group was significantly lower than that in the control group （χ2=5.165，P<0.05）.Conclusion　PNFA for the treatment of intertrochanteric fracture has the advantages of preonunced effect，less trauma and intraoperative bleeding，shorter operation time and hospitalization time，stable internal fixation，faster fracture healing and fewer complications.
