摘要:
目的 观察子宫内膜癌(EC)组织中人附睾蛋白4(HE4)和糖类抗原125(CA125)的表达,探讨其与EC发生、发展的关系。方法 选择2015年12月至2016年12月宜宾市第一人民医院收治的35例EC患者的EC组织和癌旁组织标本,采用免疫组织化学法检测EC组织和癌旁组织中HE4、CA125的表达,分析HE4、CA125表达与EC患者临床病理特征的关系,以及HE4与CA125的相关性。结果 EC组织中HE4、CA125阳性表达率分别为82.9%(29/35)、74.3%(26/35),癌旁组织中HE4、CA125阳性表达率分别为11.4%(4/35)、14.3%(5/35),EC组织中HE4和CA125阳性表达率显著高于癌旁组织(χ2=35.831、25.533,P<0.05)。Ⅲ、Ⅳ期EC组织中HE4、CA125阳性表达率分别为100.0%(20/20)、90.0%(18/20),Ⅰ、Ⅱ期EC组织中HE4、CA125阳性表达率分别为60.0%(9/15)、53.3%(8/15),Ⅲ、Ⅳ期EC组织中HE4、CA125阳性表达率显著高于Ⅰ、Ⅱ期EC组织(χ2=9.655、4.266,P<0.05);中、低分化EC组织中HE4、CA125阳性表达率分别为100.0%(18/18)、83.3%(15/18),高分化EC组织中HE4、CA125阳性表达率分别为64.7%(11/17)、64.7%(11/17),中、低分化EC组织中HE4、CA125阳性表达率显著高于高分化EC组织(χ2=7.667、4.137,P<0.05);肌层浸润深度≥1/2的EC组织中HE4、CA125阳性表达率分别为100.0%(18/18)、94.4%(17/18),肌层浸润深度<1/2的EC组织中HE4、CA125阳性表达率分别为64.7%(11/17)、52.9%(9/17),肌层浸润深度≥1/2的EC组织中HE4、CA125阳性表达率显著高于肌层浸润深度<1/2的EC组织(χ2=7.667、7.884,P<0.05);发生淋巴结转移的EC组织中HE4、CA125阳性表达率分别为100.0%(19/19)、78.9%(15/19),无淋巴结转移的EC组织中HE4、CA125阳性表达率分别为62.5%(10/16)、68.8%(11/16),发生淋巴结转移的EC组织中HE4阳性表达率显著高于无淋巴结转移的EC组织(χ2=8.599,P<0.05),但发生淋巴结转移与无淋巴结转移的EC组织中CA125阳性表达率比较差异无统计学意义(χ2=0.473,P>0.05);EC组织中HE4、CA125的表达与患者年龄、肿瘤大小及病理类型无显著相关性(P>0.05)。EC组织中HE4表达与CA125表达呈显著正相关(r=0.608,P<0.05)。结论 HE4和CA125可能参与了EC的发生和发展,EC组织中HE4和CA125呈高表达,二者的表达水平与EC临床分期、组织分化程度及肌层浸润程度密切相关,HE4高表达还与EC淋巴结转移有关。
Abstract:
Objective To observe the expression of human epididymis protein 4(HE4) and carbohydrate antigen 125(CA125) in endometrial carcinoma(EC) tissues,and to explore the relationship between the expression of HE4 and CA125 and the occurrence and development of EC.Methods The EC and paracancerous tissue specimens of 35 EC patients were selected from December 2015 to December 2016 in the First People′s Hospital of Yibin City.The expression of HE4 and CA125 in EC tissues and paracancerous tissues was detected by immunohistochemistry.The relationship between the expression of HE4,CA125 and the clinicopathological features of EC was analyzed;and the correlation between HE4 and CA125 was analyzed too.Results The positive expression rates of HE4 and CA125 in EC tissues were 82.9%(29/35) and 74.3%(26/35);they were 11.4%(4/35) and 14.3%(5/35) in paracancerous tissues respectively;the positive expression rates of HE4 and CA125 in EC tissues were significantly higher than those in paracancerous tissues(χ2 = 35.831,25.533;P<0.05).The positive expression rates of HE4 and CA125 in stage Ⅲ and Ⅳ EC tissues were 100.0%(20/20) and 90.0%(18/20);they were 60.0%(9/15) and 53.3%(8/15) in stageⅠand Ⅱ EC tissues respectively;the positive expression rates of HE4 and CA125 in stage Ⅲ and Ⅳ EC tissues were significantly higher than those in stageⅠ and Ⅱ EC tissues(χ2 = 9.655,4.266;P<0.05).The positive expression rates of HE4 and CA125 in medium and low differentiated EC tissues were 100.0%(18/18) and 83.3%(15/18);they were 64.7%(11/17) and 64.7%(11/17) in highly differentiated EC tissues respectively;the positive expression rates of HE4 and CA125 in medium and low differentiated EC tissues were significantly higher than those in highly differentiated EC tissues(χ2=7.667,4.137;P<0.05).The positive expression rates of HE4 and CA125 in EC tissues with myometrial invasion depth≥1/2 were 100.0%(18/18) and 94.4%(17/18);they were 64.7%(11/17) and 52.9%(9/17) in EC tissues with myometrial invasion depth <1/2 respectively;the positive expression rates of HE4 and CA125 in EC tissues with myometrial invasion depth ≥1/2 were significantly higher than those in EC tissues with myometrial invasion depth<1/2(χ2=7.667,7.884;P<0.05).The positive expression rates of HE4 and CA125 in EC tissues with lymph node metastasis were 100.0%(19/19) and 78.9%(15/19);they were 62.5%(10 /16) and 68.8%(11/16) in EC tissues without lymph node metastasis respectively;the positive expression rate of HE4 in EC tissues with lymph node metastasis was significantly higher than that in EC tissues without lymph node metastasis(χ2=8.599,P<0.05),but there was no significant difference in the positive expression rate of CA125 in EC tissues with lymph node metastasis and without lymph node metastasis(χ2=0.473,P>0.05).The expression of HE4 and CA125 in EC tissues was not related to age,tumor size and pathological type(P>0.05).The expression of HE4 in EC tissues was positively correlated with the expression of CA125(r=0.608,P<0.05).Conclusion HE4 and CA125 may be involved in the development and development of EC.The expression of HE4 and CA125 is high in EC tissues,the expression level of them is closely related to clinical stage,histological differentiation and myometrial invasion depth of EC.The high expression of HE4 was also related to lymph node metastasis.
