摘要:
目的　比较自体焦痂与异种脱细胞真皮微粒皮肤移植术治疗皮肤烧伤的疗效。方法　选择2014年1月至2015年1月新乡市第二人民医院收治的皮肤烧伤患者24例，随机分为对照组和观察组，每组12例，对照组患者接受异种脱细胞真皮微粒皮肤移植术，观察组患者行自体焦痂微粒皮肤移植术，观察2组患者覆盖物外观变化、微粒皮肤存活情况及术后愈合情况。结果　观察组和对照组患者残留创面分别为（21.3±4.3）%和（34.8±3.4）%，观察组患者残留创面显著小于对照组（t=8.531,P<0.05）。观察组和对照组患者创面愈合时间分别为(66.3±13.1)d和(53.7±15.3)d，观察组患者创面愈合时间显著长于对照组（t=2.199，P<0.05）；观察组和对照组患者创面愈合率分别为（89.7±23.2）%和（70.3±21.4）%，观察组患者创面愈合率显著高于对照组（t=2.158，P<0.05）。观察组和对照组患者满意度评分分别为23.1±5.7和17.8±5.5，观察组患者满意度评分显著高于对照组（t=2.274，P<0.05）。结论　自体焦痂微粒皮肤移植术治疗皮肤烧伤创面愈合率较高，外观较好，其疗效优于异种脱细胞真皮微粒皮肤移植术。
Abstract:
Objective　To compare the effect between autologous eschar microskin grafting and heterogeneous acellular dermal matrix skin grafting in the treatment of skin burns.Methods　Twenty-four patients with skin burns were selected in the Second People′s Hospital of Xinxiang City from January 2014 to January 2015,then the patients were randomly divided into control group and observation group,twelve patients in each group.The patients in control group were treated with heterogeneous acellular dermal matrix skin grafting,and the patients in observation group were treated with autologous eschar microskin grafting.The apparent changes of the slipcover,microskin survival and postoperative healing were observed in the two groups.Results　The residual wound in observation group and control group was (21.3±4.3)% and (34.8±3.4)% respectively,the residual wound in observation group was significantly less than that in control group(t=8.531,P<0.05).The wound healing time in observation group and control group was (66.3±13.1) d and (53.7±15.3) d respectively,the wound healing time in observation group was significantly longer than that in control group(t=2.199,P<0.05).The wound healing rate in observation group and control group was (89.7±23.2)% and (70.3±21.4)% respectively,the wound healing rate in observation group was significantly higher than that in control group(t=2.158，P<0.05).The satisfaction score of the patients in observation group and control group was 23.1±5.7 and 17.8±5.5 respectively,the satisfaction score of the patients in observation group was significantly higher than that in control group(t=2.274，P<0.05).Conclusion　Autologous eschar microskin grafting in the treatment of skin burns can obtain high wound healing rate and good appearance.The effect of autologous eschar microskin grafting in the treatment of skin burns is superior to that of heterogeneous acellular dermal matrix skin grafting.
