摘要:
目的　探讨CD7抗原表达与急性髓系白血病（AML）临床特点、免疫分型、细胞遗传学改变及预后关系。方法　选择2012年1月至2014年9月濮阳市安阳地区医院收治的AML患者154例，对CD7阳性AML与CD7阴性AML患者的临床特点、免疫表型、染色体分析及治疗效果进行比较。结果　154例患者中，CD7阴性表达123例（CD7-组），CD7阳性表达31例（CD7+组）。CD7+组与CD7-组患者性别、年龄、肝脾大比例、白细胞计数、血小板计数及骨髓原始细胞计数比较差异均无统计学意义（P>0.05）。2组患者CD13、CD33、髓过氧化物酶、CD117及CD38阳性表达率比较差异均无统计学意义（P>0.05），但CD7+组患者CD34、人白细胞DR抗原（HLA-DR）阳性表达率显著高于CD7-组（P<0.05）。CD7+组多对应预后中等及预后差的染色体核型。CD7-组与CD7+组患者治疗缓解率分别为68.3%和40.9%；CD7+组治疗缓解率显著低于CD7-组（P<0.05）。结论　CD7阳性AML常伴CD34及HLA-DR等早期抗原高表达，相对应的染色体核型为预后中等或差者，临床治疗缓解率低；CD7抗原表达是AML预后不良的一项指标。
Abstract:
Objective　To investigate the relationship between CD7 antigen expression and clinical features，cytogenetic features，prognosis of acute myeloid leukemia（AML）.Methods　A total of 154 patients with AML in Anyang Regional Hospital from January 2012 to September 2014 were selected.The clinical characteristics，immune phenotype，chromosome analysis and therapeutic effect were compared between CD7 positive patients and CD7 negative patients.Results　Among the 154 cases，CD7 expression was negative in 123 cases（CD7-group），CD7 expression was positive in 31 cases（CD7+group）.There was no significant difference in gender，age，hepatosplenomegaly proportion，white blood cell count，blood platelet count and bone marrow initial cells between CD7+group and CD7-group（P>0.05）.There was no significant difference in the positive expression rate of CD13，CD33，myeloperoxidase，CD117 and CD38 between the two groups（P>0.05）.But the positive expression rate of CD34 and human leukocyte antigen DR（HLA-DR） in CD7+group was significantly higher than that in CD7-group（P<0.05）.The karyotypes of CD7+group most corresponded poor and moderate prognosis.The remission rates in CD7-group and CD7+group was 68.3% and 40.9% respectively，the remission rates in CD7+group was significantly lower than that in CD7-group（P<0.05）.Conclusion　CD7 positive AML has high expression of early antigen CD34 and HLA-DR，it corresponds the karyotypes of poor and moderate prognosis，and the remission rate is low.CD7 antigen expression is an index of poor prognosis of AML.
