摘要:
目的 探讨不同性质肺结节患者心肺功能的特点,分析肺结节性质与心肺功能的相关性。方法 选择2019年1月至2020年12月河南省人民医院收治的468例肺结节患者为研究对象,根据肺结节组织病理检查结果将患者分为良性组(n=248)和恶性组(n=220);另选择同期体检健康者100例为对照组。所有受试者进行活动平板运动试验和肺通气、肺弥散功能检测,分析不同性质肺结节患者的心功能、肺功能的差异性和相关性。结果 3组受试者的静息收缩压、静息舒张压、最高收缩压、最高舒张压比较差异无统计学意义(P>0.05);3组受试者的最高心率、代谢当量、运动时间比较差异有统计学意义(P<0.05),良性组和恶性组受试者的最高心率、代谢当量、运动时间显著低于对照组(P<0.05),恶性组受试者的最高心率、代谢当量、运动时间显著低于良性组(P<0.05)。对照组、良性组和恶性组受试者活动平板运动试验阳性率分别为0.00%(0/100)、8.47%(21/248)、19.55%(43/220),良性组和恶性组受试者活动平板运动试验阳性率显著高于对照组(χ2=2.114、1.883,P<0.05),恶性组受试者活动平板运动试验阳性率显著高于良性组(χ2=1.508,P<0.05)。3组受试者的肺一氧化碳弥散量、用力肺活量(FVC)、第1秒用力呼气量(FEV1)/FVC、用力呼气时间(FET)比较差异无统计学意义(P>0.05);良性组和恶性组受试者的呼气流量峰值(PEF)、FEV1、用力呼出50%肺活量时的瞬间呼气流量(FEF50)、用力呼出75%肺活量时的瞬间呼气流量(FEF75)、最大呼气中期流量(MMEF)显著低于对照组(P<0.05),恶性组受试者的PEF、FEF75显著低于良性组(P<0.05),恶性组与良性组受试者的FEV1、FEF50、MMEF比较差异无统计学意义(P>0.05)。结论 肺结节患者心肺功能有不同程度的下降,恶性肺结节患者心肺功能下降更显著,心肺功能检查可为鉴别结节性质、制定治疗方案、评估预后提供参考。
Abstract:
Objective To investigate the characteristics of cardiopulmonary function in patients with different nature of pulmonary nodules,and to analyze the correlation between the nature of nodule and cardiopulmonary function.Methods A total of 468 patients with pulmonary nodules treated in Henan Provincial People′s Hospital from January 2019 to December 2020 were selected as the research subjects,and the patients were divided into the benign group (n=248) and malignant group (n=220) according to the histopathological examination results of pulmonary nodules.Another 100 healthy persons in the same period were selected as the control group.All subjects underwent treadmill exercise test,pulmonary ventilation and pulmonary diffusion function test,and the differences and correlation of cardiopulmonary function in patients with different nature of pulmonary nodules were analyzed.Results There was no significant difference in resting systolic blood pressure,resting diastolic blood pressure,maximum systolic blood pressure and maximum diastolic blood pressure among the three groups (P>0.05).There were significant differences in the maximum heart rate,metabolic equivalent and exercise time among the three groups (P<0.05).The maximum heart rate,metabolic equivalent and exercise time of subjects in the benign group and malignant group were significantly lower than those in the control group (P<0.05).The maximum heart rate,metabolic equivalent and exercise time of subjects in the malignant group were significantly lower than those in the benign group (P<0.05).The positive rate of treadmill exercise test in the control group,benign group and malignant group was 0.00% (0/100),8.47% (21/248) and 19.55% (43/220),respectively.The positive rate of treadmill exercise test in the benign group and malignant group was significantly higher than that in the control group (χ2 = 2.114,1.883;P<0.05).The positive rate of treadmill exercise test in the malignant group was significantly higher than that in the benign group (χ2=1.508,P<0.05).There was no significant difference in diffusion capacity for carbon monoxide of lung,forced vital capacity (FVC),forced expiratory volume in the first second (FEV1)/FVC and forced expiratory time (FET) among the three groups (P>0.05).The peak expiratory flow (PEF),FEV1,forced expiratory flow at 50% vital (FEF50),forced expiratory flow at 75% vital (FEF75) and maximal midexpiratory flow (MMEF) in the benign group and malignant group were significantly lower than those in the control group (P<0.05).The PEF,FEF75 of subjects in the malignant group were significantly lower than those in the benign group (P<0.05);there was no significant difference in FEV1,FEF50 and MMEF between the malignant group and benign group (P>0.05).Conclusion The cardiopulmonary function in patients with pulmonary nodules decreases in varying degrees,especially in patients with malignant pulmonary nodules.The cardiopulmonary function examination can provide reference for identifying the nature of nodules,formulating treatment plans and evaluating prognosis.
