摘要:
目的　探讨膝单髁置换术治疗膝单间室重度骨性关节炎的临床疗效。方法　回顾性分析2015年7月至2019年7月新乡医学院第三附属医院收治的100例膝单间室重度骨性关节炎患者的临床资料，依据手术方式将患者分为膝单髁置换术组和全膝关节置换术组，每组50例。比较2组患者的手术时间、住院时间及术中出血量；术前及术后6个月，使用美国纽约特种外科医院膝关节（HSS）评分评估2组患者膝关节功能改善情况，使用关节活动度量角器测定2组患者膝关节活动度，应用视觉模拟评分法（VAS）评估2组患者疼痛程度；比较2组患者术后并发症发生率。结果　2组患者手术时间比较差异无统计学意义（P＞0.05）；膝单髁置换术组患者住院时间短于全膝关节置换术组，术中出血量低于全膝关节置换术组（P＜0.05）。2组患者术前HSS、VAS评分及关节主动活动度比较差异无统计学意义（P＞0.05）；术后6个月，2组患者HSS评分及关节主动活动度均高于术前（P＜0.05），VAS评分低于术前（P＜0.05）；且膝单髁置换术组患者HSS评分及关节主动活动度均高于全膝关节置换术组（P＜0.05），VAS评分低于全膝关节置换术组（P＜0.05）。2组患者术后均未发生并发症。结论　膝单髁置换术治疗膝单间室重度骨性关节炎能够促进患者术后膝关节功能恢复，显著缓解疼痛症状。
Abstract:
Objective　To explore the efficacy of unicompartmental knee arthroplasty (UKA) in the treatment of severe unicompartment knee osteoarthritis.Methods　　The clinical data of 100 patients with severe unicompartment knee osteoarthritis who admitted in the Third Affiliated Hospital of Xinxiang Medical University from July 2015 to July 2019 were retrospectively analyzed,and they were divided into UKA group and total knee arthroplasty（TKA） group,with 50 patients in each group.The operative time,hospital stay and intraoperative blood loss were compared between the two groups.Before and six months after operation, the knee joint function improvement of all patients was evaluated by the hospital for special surgery (HSS) score; the angle of knee joint motion of all patients was measured by joint range of motion protractor;the pain degree of all patients was evaluated by the visual analogue scales(VAS).And the incidence of postoperative complications was compared between the two groups.Results　There was no significant difference in operation time between the two groups(P>0.05);the hospital stays of patients in the UKA group was shorter than those in the TKA group,and the intraoperative bleeding volume of patients in UKA group was lower than that in the TKA group (P<0.05).There was no significant difference in the HSS,VAS scores and joint active range of motion between the two groups（P＞0.05）.At six months after operation,the HSS scores and active range of motion of patients in the two groups were higher than those before operation（P＜0.05）,and the VAS was lower than that before operation（P＜0.05）.At six months after operation,the HSS scores and active range of motion of patients in the UKA group were higher than those in the TKA group（P＜0.05）,and the VAS was lower than that in the TKA group（P＜0.05）.No postoperative complications occurred in the two groups.Conclusion　Treating severe unicompartment knee osteoarthritis with UKA can promote the recovery of knee function,significantly relieve pain symptoms .
