摘要:
目的 探讨鹿茸多肽(VAP)对大鼠心肌缺血再灌注损伤的保护作用机制及其对血清心肌肌钙蛋白T(cTnT)、乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)水平的影响。方法 将40只雄性Sprague Dawley大鼠随机分为假手术组、模型组、低剂量VAP组和高剂量VAP组,每组10只。模型组、低剂量VAP组和高剂量VAP组大鼠采用左冠状动脉前降支结扎法建立心肌缺血再灌注模型,假手术组大鼠仅分离左冠状动脉前降支而不结扎。低剂量VAP组和高剂量VAP组大鼠分别术前连续7 d 腹腔注射VAP 5、20 mg·kg-1·d-1,假手术组和模型组大鼠分别注射等量的生理盐水。模型建立成功后眼眶取血,采用比色法检测各组大鼠血清cTnT水平,使用生物化学分析仪检测血清LDH和CK-MB水平;眼眶取血后取出大鼠心脏,苏木精-伊红染色和Masson染色观察心肌组织病理学变化,计算心肌梗死面积和胶原附着面积;Western blot 法检测心肌组织中核转录因子E2相关因子2(Nrf2)、血红素加氧酶-1(HO-1)蛋白的表达。结果 模型组大鼠心肌梗死面积和胶原附着面积大于假手术组(P<0.05),低、高剂量VAP组大鼠心肌梗死面积和胶原附着面积显著小于模型组(P<0.05),高剂量VAP组大鼠心肌梗死面积和胶原附着面积显著小于低剂量VAP组(P<0.05)。与假手术组比较,模型组和低、高剂量VAP组大鼠血清cTnT、CK-MB、LDH水平及心肌组织中Nrf2、HO-1蛋白表达水平显著升高(P<0.05);与模型组比较,低、高剂量VAP组大鼠血清cTnT、CK-MB、LDH水平显著降低,心肌组织中Nrf2、HO-1蛋白表达水平显著升高(P<0.05);高剂量VAP组大鼠血清cTnT、CK-MB、LDH水平显著低于低剂量VAP组,心肌组织中Nrf2、HO-1蛋白表达水平显著高于低剂量VAP组(P<0.05)。结论 VAP预处理对心肌缺血再灌注损伤具有保护作用,可显著降低血清LDH、CK-MB及cTnT等心肌酶的水平,该作用可能与VAP激活Nrf2/抗氧化物反应元件通路有关。
Abstract:
Objective To investigate the protective mechanism of velvet antler polypeptides (VAP) on myocardial ischemia-reperfusion injury in rats and its effect on the levels of serum cardiac troponin T(cTnT),lactate dehydrogenase(LDH)and creatine kinase isoenzyme(CK-MB).Methods Forty Sprague Dawley rats were randomly divided into sham operation group,model group,low dose VAP group and high dose VAP group,with 10 rats in each group.The rat models of myocardial ischemia reperfusion were established by ligating the anterior descending branch of the left coronary artery in the model group,the low dose VAP group and the high dose VAP group.But in the sham operation group,only the anterior descending branch of the left coronary artery of rats was isolated without ligation.The rats in the low dose VAP group and the high dose VAP group were intraperitoneally injected with VAP 5,20 mg·kg-1· d-1 for 7 days before surgery,respectively.The rats in the sham operation group and the model group were injected with the same amount of normal saline.Subsequently,the blood was taken from the eyelids,and the level of serum cTnT in each group were detected by colorimetry.The serum LDH and CK-MB levels were detected by biochemical analyzer.The heart of rats was removed.The pathological changes of myocardium were observed by hematoxylin-eosin staining and Masson staining,and the myocardial infarction area and collagen attachment area were calculated.The expression of nuclear transcription factor erythroid 2-related factor 2 (Nrf2) and heme oxygenase-1 (HO-1) proteins were detected by Western blot.Results The area of myocardial infarction and collagen adhesion of rats in the model group were greater than those in the sham operation group(P<0.05).The area of myocardial infarction and collagen adhesion of rats in the low dose VAP group and high dose VAP group were significantly less than those in the model group (P<0.05),and the area of myocardial infarction and collagen adhesion of rats in the high dose VAP group were significantly less than those in the low dose VAP group (P<0.05).Compared with the sham operation group,the serum cTnT,CK-MB,LDH levels and Nrf2,HO-1 protein expressions in myocardium were higher in the dose VAP group and the high dose VAP group(P<0.05).Compared with the model group,the serum cTnT,CK-MB,LDH levels were lower and Nrf2,HO-1 protein expressions in myocardium were higher in the dose VAP group and the high dose VAP group(P<0.05).The levels of serum cTnT,CK-MB and LDH in the high dose VAP group were significantly lower than those in the low dose VAP group,and the expression of Nrf2 and HO-1 protein in the myocardial tissues were significantly higher than those in the low dose VAP group (P<0.05).Conclusions VAP preconditioning has an obvious protective effect on myocardial ischemia-reperfusion induced myocardial injury,and can significantly reduce the levels of LDH,CK-MB and cTnT,which may be related to the activation of Nrf2/antioxidant response elements pathway by VAP.
