摘要:
目的 探讨胸段硬膜外阻滞(TEB)对急性缺氧性肺损伤大鼠炎性因子水平的影响。方法 将30只健康雄性Wistar大鼠随机分为对照组、缺氧组、TEB组。各组大鼠均先行胸段硬膜外腔置管并留置,TEB组大鼠经胸段硬膜外腔给予5 g·L-1罗哌卡因(0.125 mL·kg-1),对照组与缺氧组大鼠经胸段硬膜外腔给予等量的生理盐水。TEB建立成功10 min后各组大鼠行气管插管、机械通气,缺氧组与TEB组大鼠吸入含体积分数14%氧的氧氮混合气体,直至动脉血氧合指数<300,说明急性缺氧性肺损伤模型建立成功;对照组大鼠始终吸入含体积分数21%氧的氧氮混合气体。于气管插管前(T0)、急性缺氧性肺损伤模型建立成功后4 h(T1)进行动脉血气分析。急性缺氧性肺损伤模型建立成功后4 h抽取各组大鼠动脉血,然后处死大鼠,收集支气管肺泡灌洗液(BALF),采用酶联免疫吸附试验检测T0、T1时各组大鼠血清肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-6 和IL-10水平及T1时BALF中 TNF-α、IL-6、IL-10水平。结果 3组大鼠T0时动脉血氧合指数比较差异无统计学意义(P>0.05)。缺氧组和TEB组大鼠T1时动脉血氧合指数低于T0(P<0.05)。缺氧组和TEB组大鼠T1时动脉血氧合指数低于对照组(P<0.05)。TEB组大鼠T1时动脉血氧合指数高于缺氧组(P<0.05)。3组大鼠T0时血清和TNF-α、IL-6、IL-10水平比较差异无统计学意义(P>0.05),T1时血清TNF-α、IL-6、IL-10水平高于T0时(P<0.05)。缺氧组、TEB组大鼠T1时血清和BALF中TNF-α、IL-6 和IL-10水平高于对照组(P<0.05)。TEB组大鼠T1时血清和BALF中TNF-α、IL-6和IL-10水平低于缺氧组(P<0.05)。结论 TEB可减少急性缺氧性肺损伤大鼠炎性细胞因子的释放,纠正炎症反应和抗炎反应的失衡,对机体有一定的保护作用。
Abstract:
Objective To investigate the effect of thoracic epidural block (TEB) on the levels of inflammatory factors in rats with acute hypoxic lung injury.Methods Thirty healthy male Wistar rats were randomly divided into control group,hypoxia group and TEB group.The rats in each group were given thoracic epidural catheter and indwelling firstly.The rats the in the TEB group were given 5 g·L-1 ropivacaine 0.125 mL·kg-1 through epidural cavity to establish TEB model.While the rats in the control group and the hypoxia group were given the same amount of saline through epidural cavity.After the establishment of TEB model for 10 min,the rats in each group received tracheal intubation and mechanical ventilation.The rats in the hypoxia group and the TEB group inhaled oxygen-nitrogen mixture containing 14% oxygen.The arterial oxygenation index was less than 300,which indicated that the model of acute hypoxic lung injury was established successfully.The rats in the control group inhaled oxygen-nitrogen mixture containing 21% oxygen.Arterial blood gas was measured before trachea cannula(T0) and at four hours after establishment of acute hypoxic lung injury model.Four hours after the establishment of acute hypoxic lung injury model,arterial blood was taken from rats in each group and then the rats were sacrificed.The serum and bronchoalveolar lavage fluid (BALF) were collected.The levels of serum tumor necrosis factor-α (TNF-α),interleukin(IL)-6,IL-10 and the levels of TNF-α,IL-6,IL-10 in BALF at T0 and T1 were measured by enzyme-linked immunosorbent assay.Results There was no significant difference in arterial oxygenation index among the three groups at T0(P>0.05).The arterial oxygenation index of the hypoxic group and TEB group at T1 was lower than that of T0(P<0.05).The arterial oxygenation index in the hypoxia group and TEB group at T1 was lower than that in the control group (P<0.05).The arterial oxygenation index in the TEB group at T1 was higher than that in the hypoxic group (P<0.05).There was no significant difference in the level of serum TNF- α,IL-6 and IL-10 among the three groups at T0 (P>0.05),but the levels of serum TNF-α,IL-6 and IL-10 at T1 in the three groups were higher than those at T0(P<0.05).The levels of TNF-α,IL-6 and IL-10 in serum and BALF of the hypoxia group and the TEB group at T1 were higher than those in the control group (P<0.05).The levels of TNF-α,IL-6 and IL-10 in serum and BALF at T1 in the hypoxic group were lower than those in the hypoxic group (P<0.05).Conclusion TEB can reduce the release of inflammatory cytokines and correct the imbalance between inflammatory reaction and anti-inflammatory reaction in rats with acute hypoxic lung injury.
