摘要:
目的 探讨胸段硬膜外阻滞(TEB)对急性心肌梗死(AMI) 大鼠心肌细胞凋亡及凋亡蛋白Caspase-3的影响。方法 将36只健康雄性Wistar 大鼠随机分为假手术组、模型组和罗哌卡因组,每组12只。造模前罗哌卡因组大鼠经硬膜外腔注入5 g·L-1罗哌卡因(0.125 mL·kg-1)进行胸段硬膜外阻滞,假手术组和模型组大鼠硬膜外腔注入等量生理盐水。胸段硬膜外阻滞成功后,罗哌卡因组、模型组大鼠采用结扎左冠状动脉前降支的方法制备AMI模型,假手术组大鼠仅穿线不结扎左冠状动脉前降支。记录各组大鼠开胸前(T0)、冠状动脉结扎前(T1)、冠状动脉结扎后3 h(T2)的心率(HR)和平均动脉压(MAP);采用酶联免疫吸附试验检测各组大鼠T0、T1、T2时血清心肌肌钙蛋白I(cTnI)水平;Western blot法检测各组大鼠心肌组织中Caspase-3蛋白相对表达量。结果 假手术组大鼠T0、T1、T2时HR、MAP两两比较差异均无统计学意义(P>0.05),模型组和罗哌卡因组大鼠HR、MAP在T2时显著低于T0、T1时(P<0.05);模型组和罗哌卡因组大鼠HR、MAP在T0和T1时比较差异无统计学意义(P>0.05)。3组大鼠T0、T1时HR、MAP比较差异均无统计学意义(P>0.05);T2时,模型组和罗哌卡因组大鼠HR、MAP显著低于假手术组(P<0.05);罗哌卡因组大鼠HR、MAP显著高于模型组(P<0.05)。假手术组大鼠血清cTnI水平在T0、T1、T2时比较差异均无统计学意义(P>0.05);模型组和罗哌卡因组大鼠血清cTnI水平在T2时显著高于T0、T1时(P<0.05);模型组和罗哌卡因组大鼠血清cTnI水平在T0和T1时比较差异无统计学意义(P>0.05)。T0、T1时,3组大鼠血清cTnI水平比较差异均无统计学意义(P>0.05);T2时,模型组和罗哌卡因组大鼠血清cTnI水平显著高于假手术组(P<0.05);T2时,罗哌卡因组大鼠血清cTnI水平显著低于模型组(P<0.05)。罗哌卡因组、模型组大鼠心肌组织中Caspase-3蛋白表达水平显著高于假手术组(P<0.01);罗哌卡因组大鼠心肌组织中Caspase-3蛋白表达水平低于模型组(P<0.05)。结论 TEB能够减轻AMI后的心肌细胞凋亡,对缺血或梗死心肌有一定的保护作用。
Abstract:
Objective To explore the effect of thoracic epidural block (TEB) on apoptosis of myocardial cells and expression of caspase-3 in myocardial cells of rats with acute myocardial infarction (AMI).Methods Thirty-six healthy male Wistar rats were randomly divided into sham operation group,model group and ropivacaine group,with 12 rats in each group.Before modeling,the rats in the ropivacaine group were injected with 5 g·L-1 ropivacaine(0.125 mL·kg-1) via epidural space for TEB;the rats in the sham operation group and model group were injected with the same amount of saline via epidural space.After the TEB was established successfully,the AMI modeling was established by ligating the anterior descending branch of the left coronary artery of rats in the model group and ropivacaine group;the rats in the sham operation were only threaded,the anterior descending branch of the left coronary artery was not ligated.The heart rate (HR),mean arterial pressure (MAP) of rats before thoracotomy (T0),before coronary ligation (T1) and 3 hours after coronary ligation(T2)were recorded;the serum levels of cardiac troponin I (cTnI) of rats at T0,T1,T2 were detected by enzyme linked immunosorbent assay;the relative expression of Caspase-3 protein in cardiac muscle tissue was detected by Western blot.Results There was no statistic difference in the HR and MAP of rats among the time point of T0,T1,T2 in sham operation group(P>0.05);the HR and MAP of rats at T2 were significantly lower than those at T0,T1in the model group and ropivacaine group (P<0.05);and there was no statistic difference in the HR and MAP between T0 and T1 in the model group and ropivacaine group (P>0.05).There was no statistic difference in the HR and MAP of rats among the three groups at the time point of T0 and T1 (P>0.05);the HR and MAP of rats in the model group and ropivacaine group were significantly lower than those in the sham operation group at T2 (P<0.05);the HR and MAP of rats in the ropivacaine group were significantly higher than those in the model group at T2 (P<0.05).There was no statistic difference in the serum levels of cTnI of rats among the time point of T0,T1,T2 in sham operation group(P>0.05);the serum levels of cTnI of rats at T2 were significantly higher than those at T0,T1 in the model group and ropivacaine group (P<0.05);and there was no statistic difference in the serum levels of cTnI of rats between T0 and T1 in the model group and ropivacaine group (P>0.05).There was no statistic difference in the serum levels of cTnI of rats among the three groups at the time point of T0 and T1 (P>0.05);the serum levels of cTnI of rats in the model group and ropivacaine group were significantly higher than those in the sham operation group at T2 (P<0.05);the serum levels of cTnI of rats in the ropivacaine group were significantly lower than those in the model group at T2 (P<0.05).The expression of Caspase-3 protein in myocardium tissue of rats in ropivacaine group and model group was significantly higher than that in the sham operation group(P<0.01);the expression of Caspase-3 protein in myocardium tissue of rats in the ropivacaine group was significantly lower that in the model group(P<0.05).Conclusion TEB can alleviate apoptosis of myocardial cells after AMI,and it has a protective effect on ischemic or infarcted myocardium.
