摘要:
目的 探讨脑干听觉诱发电位(BAEP)和闪光视觉诱发电位(FVEP)检测在新生儿缺氧缺血性脑病(HIE)中的应用价值。方法 选择2016年5月至2017年6月新乡市中心医院收治的确诊为缺氧缺血性脑病的新生儿40例为HIE组,另选择同期正常足月新生儿20例作为对照组,分别于出生后20 d对2组新生儿进行BAEP和FVEP检测,并测定Ⅴ波阈值。结果 HIE组新生儿BAEP中Ⅰ、Ⅲ、Ⅴ波的潜伏期及Ⅰ~Ⅲ、Ⅲ~Ⅴ、Ⅰ~Ⅴ峰间期显著长于对照组(P<0.05),HIE组新生儿BAEP中Ⅰ、Ⅲ、Ⅴ波的波幅显著低于对照组(P<0.05),HIE组新生儿BAEP中Ⅴ波阈值显著高于对照组(P<0.05),HIE组新生儿FVEP主波潜伏期显著长于对照组(P<0.05)。结论 BAEP和FVEP联合检测可以早期准确评估HIE新生儿脑损伤及听力损伤程度,对新生儿HIE的早期诊断、病情评估及指导治疗具有重要意义。
Abstract:
Objective To investigate the value of brain stem auditory evoked potential (BAEP) and flash visual evoked potential (FVEP) in neonatal hypoxic-ischemic encephalopathy (HIE).Methods A total of 40 neonatus with HIE in Xinxiang Central Hospital from May 2016 to June 2017 were selected as the HIE group,and 20 normal full-term newborns were selected as the control group at the same time.The newborns in the two groups were performed with BAEP and FVEP,and the threshold of wave Ⅴ was measured on the 20th day after birth.Results The latency period of wave Ⅰ,Ⅲ,Ⅴ and the interpeak latency of Ⅰ-Ⅲ,Ⅲ-Ⅴ,Ⅰ-Ⅴ of BAEP in HIE group were significantly longer than those in the control group (P<0.05).The amplitude of wave Ⅰ,Ⅲ and Ⅴ of BAEP in HIE group was significantly lower than that in the control group (P<0.05).The threshold of wave Ⅴ of BAEP in HIE group was significantly higher than that in the control group (P<0.05).The latency period of FVEP main wave in HIE group was significantly longer than that in the control group (P<0.05).Conclusion The combined detection of BAEP and FVEP can early and accurately assess the degree of brain damage and hearing loss in HIE neonatus,and it is important to the early diagnosis and disease assessment and treatment guidance of neonatal HIE.
