摘要:
目的　探讨血清C-反应蛋白（CRP）、全血血小板与淋巴细胞比值（PLR）和血浆纤维蛋白原（FIB）联合检测对早期糖尿病肾病（EDN）的诊断价值。
方法　选择2022年3月至11月河南省人民医院收治的55例EDN患者（EDN组）及55例单纯2型糖尿病（T2DM）患者（T2DM组）为观察对象，并选择同期在本院体检健康者55例为对照组，比较3组受试者血清CRP、血浆FIB水平及全血PLR，评估血清CRP、全血PLR和血浆FIB单独及联合检测对EDN的诊断价值。
结果　3组受试者血清CRP、血浆FIB水平及全血PLR比较差异均有统计学意义（F=9.145、6.891、35.358，P<0.05）。T2DM组和EDN组患者血清CRP水平、血浆FIB水平及全血PLR显著高于对照组（P<0.05）。EDN组患者血清CRP、血浆FIB水平及全血PLR显著高于T2DM组（P<0.05）。血清CRP、血浆FIB、全血PLR单独及三者联合检测诊断EDN的灵敏度分别为81.25%、43.75%、70.83%、89.58%，特异度分别为76.13%、92.06%、86.34%、70.51%，曲线下面积分别为0.83、0.68、0.82、0.87。
结论　EDN患者具有较高的血清CRP水平、血浆FIB水平及全血PLR，血清CRP、血浆FIB和全血PLR联合检测对EDN具有较高的诊断价值。
Abstract:
Objective　To explore the diagnostic value of serum C-reactive protein (CRP),whole blood platelet to lymphocyte (PLR) and plasma fibrinogen (FIB) in early diabetic nephropathy (EDN).
Methods　Fifty-five patients with EDN(EDN group) and 55 patients with type 2 diabetes mellitus（T2DM） （T2DM group） admitted to Henan People′s Hospital from March to November 2022 were selected as observation objects,and 55 healthy subjects were selected as the control group.The serum CRP,whole blood PLR and plasma FIB of the subjects among the three groups were compared,and the diagnostic value of serum CRP,whole blood PLR and plasma FIB alone and in combination for EDN were evaluated.
Results　There were statistically significant differences in serum CRP,plasma FIB levels and whole blood PLR of subjects among the three groups (F=9.145,6.891,35.358;P<0.05).The serum CRP level,plasma FIB level and whole blood PLR of patients in the T2DM group and EDN group were significantly higher than those in the control group (P<0.05);the serum CRP level,plasma FIB level and whole blood PLR of patients in the EDN group were significantly higher than those in the T2DM group (P<0.05).The sensitivity of serum CRP,plasma FIB,whole blood PLR alone and in combination detection in diagnosing EDN were 81.25%,43.75%,70.83% and 89.58%,respectively;and the specificity was 76.13%,92.06%,86.34% and 70.51%,respectively;the area under the curve was 0.83,0.68,0.82 and 0.87,respectively.
Conclusion　EDN patients have high levels of serum CRP,plasma FIB,and whole blood PLR;the combined detection of serum CRP,plasma FIB,and whole blood PLR has high diagnostic value for EDN.
