摘要:
目的探讨2型糖尿病(T2DM)患者合并甲状腺功能减退症的危险因素。
方法选择2017年1月至2018年12月湖南省人民医院收治的596例T2DM患者为研究对象。根据是否合并甲状腺功能减退症将患者分为合并甲减组和未合并甲减组,其中合并甲减组62例(10.4%),未合并甲减组534例(89.6%)。通过查阅病历收集2组患者的临床资料,包括性别、年龄、体质量指数(BMI)、糖尿病病程、既往降糖方案以及空腹血糖(FPG)、空腹C肽(FCP)、餐后C肽(PCP)、糖化血红蛋白(HbAlc)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平,采用改良HOMA公式评估稳态模型胰岛素抵抗指数[HOMA-IR(CP)]。采用二元logistic回归分析T2DM患者合并甲状腺功能减退症的危险因素。
结果T2DM患者的性别、年龄、糖尿病病程、既往降糖方案、FPG、FCP、PCP、HbAlc、TC、TG、HOMA-IR(CP)与合并甲状腺功能减退症有关(P<0.05);T2DM患者的BMI、LDL-C、HDL-C与合并甲状腺功能减退症无关(P>0.05)。Logistic回归分析显示,女性、年龄>70岁、FPG、HbAlc、TG、HOMA-IR(CP)是T2DM患者合并甲状腺功能减退症的危险因素(P<0.05),服用二甲双胍是T2DM患者合并甲状腺功能减退症的保护因素(P<0.05)。
结论女性、年龄>70岁、FPG、HbAlc、TG、HOMA-IR(CP)是T2DM患者合并甲状腺功能减退症的危险因素,服用二甲双胍是T2DM患者合并甲状腺功能减退症的保护因素。
Abstract:
ObjectiveTo investigate the risk factors of hypothyroidism in patients with type 2 diabetes mellitus(T2DM).MethodsA total of 596 patients with T2DM admitted to Hunan People′s Hospital from January 2017 to December 2018 were selected as research subjects.The patients were divided into the combined hypothyroidism group (n=62)and non-combined hypothyroidism group (n=534)according to whether they had combined with hypothyroidism.Relevant clinical data of patients in the two groups were collected by consulting medical records,including gender,age,body mass index (BMI),course of diabetes,previous hypoglycemic program and fasting blood glucose(FPG),fasting C-peptide (FCP),postprandial C-peptide (PCP),hemoglobin Alc (HbAlc),total cholesterol (TC),triacylglycerol (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C) levels.The Homeostasis model assessment for insulin resistance(C-peptide)[HOMA-IR (CP)] of patients in the two groups was evaluated by the modified HOMA formula.The risk factors of hypothyroidism in T2DM patients were analyzedby binary logistic regression.Results The sex,age,course of diabetes,previous hypoglycemic program,FPG,FCP,PCP,HbAlc,TC,TG,HOMA-IR (CP) were associated with hypothyroidism in T2DM patients (P<0.05).BMI,LDL-C and HDL-C were not associated with hypothyroidism in T2DM patients (P>0.05).Logistic regression analysis showed that women,age>70 years old,FPG,HbAlc,TG,HOMA-IR (CP) were the risk factors of hypothyroidism in T2DM patients (P<0.05),and taking metformin was the protective factor of hypothyroidism in T2DM patients (P<0.05).ConclusionFemale,age>70 years old,FPG,HbAlc,TG,HOMA-IR(CP) are risk factors of hypothyroidism in T2DM patients,and taking metformin is a protective factor of hypothyroidism in T2DM patients.
