摘要:
目的 探讨甲泼尼龙联合阿奇霉素序贯疗法治疗儿童难治性肺炎支原体肺炎(RMPP)的临床效果。方法 选择2017年8月至2018年8月驻马店市第一人民医院收治的100例RMPP患儿为研究对象,根据治疗方法将患儿分为对照组和观察组,每组50例。对照组患儿给予阿奇霉素治疗,观察组患儿给予阿奇霉素联合甲泼尼龙序贯治疗;2组患者均治疗7 d为1个疗程,共治疗3个疗程。观察2组患儿咳嗽消失时间、发热消退时间及肺部啰音消失时间;分别于治疗前后检测患儿血清免疫球蛋白M(IgM)、C反应蛋白(CRP)和降钙素原(PCT)水平,治疗结束后评估2组患儿临床疗效,并观察治疗期间患儿不良反应。结果 观察组患儿咳嗽消失时间、发热消退时间、肺部啰音消失时间均显著短于对照组(P<0.05)。观察组和对照组患儿总有效率分别为94.0%(47/50)、78.0%(39/50),观察组患儿总有效率显著高于对照组(χ2=5.316,P<0.05)。2组患儿治疗前血清IgM、CRP、PCT水平比较差异无统计学意义(P>0.05),2组患儿治疗后血清IgM、CRP、PCT水平显著低于治疗前(P<0.05);治疗后,观察组患儿血清IgM、CRP、PCT水平显著低于对照组(P<0.05)。观察组和对照组患儿不良反应发生率分别为6.0%(3/50)、20.0%(10/50),观察组患儿不良反应发生率显著低于对照组(χ2=4.332,P<0.05)。结论 甲泼尼龙联合阿奇霉素序贯疗法可有效减轻RMPP患儿机体炎症反应,调节机体免疫反应,缓解患儿临床症状,且不良反应较少。
Abstract:
Objective To investigate the clinical effect of methylprednisolone combined with azithromycin sequential therapy in the treatment of refractory mycoplasma pneumoniae pneumonia (RMPP) in children.Methods A total of 100 children with RMPP admitted to the First People′s Hospital of Zhumadian City from August 2017 to August 2018 were selected as the research subjects,and the children were divided into observation group and control group according to the treatment methods,with 50 cases in each group.The children in the control group were treated with azithromycin,and the children in the observation group were given azithromycin combined with methylprednisolone sequential treatment.All patients were treated for 7 days as a course of treatment,and a total of 3 course.The extinction time of cough,fever and pulmonary rales were observed.The levels of serum immunoglobulin M (IgM),C-reactive protein (CRP) and procalcitonin (PCT) were detected before and after treatment.The clinical effect of the children in the two groups was evaluated after the treatment,and the adverse reactions during the treatment were observed.Results The extinction time of cough,fever and pulmonary rales in the observation group were significantly shorter than those in the control group (P<0.05).The total effective rate in the observation group and the control group were 94.0% (47/50) and 78.0% (39/50),respectively.The total effective rate in the observation group was significantly higher than that in the control group (χ2=5.316,P<0.05).There was no significant difference in serum IgM,CRP and PCT levels between the two groups before treatment (P>0.05).The levels of serum IgM,CRP and PCT in the two groups after treatment were significantly lower than those before treatment (P<0.05).The levels of serum IgM,CRP and PCT in the observation group were significantly lower than those in the control group after treatment (P<0.05).The incidence of adverse reactions in the observation group and the control group were 6.0% (3/50) and 20.0% (10/50),respectively.The incidence of adverse reactions in the observation group was significantly lower than that in the control group (χ2=4.332,P<0.05).Conclusion Methylprednisolone combined with azithromycin sequential therapy can effectively reduce the body inflammatory response,regulate the immune response,and relieve the clinical symptoms of children with RMPP.
