摘要:
目的 探讨生长抑素对老年脓毒症患者肠黏膜屏障功能的影响。方法 选择2017年4月至2018年3月新乡医学院第三附属医院收治的36例老年脓毒症患者为研究对象,根据治疗方法分为观察组和对照组,每组18例。对照组患者给予抗生素、液体复苏、抑制胃酸分泌及低分子肝素等常规治疗,观察组患者在常规治疗基础上给予生长抑素6 mg,溶于60 mL生理盐水中,24 h持续静脉泵入,共治疗1周。2组患者分别于治疗前及治疗3 、7 d采用胶体免疫层析法检测血清降钙素原(PCT)水平,采用动态浊度法检测血清细菌内毒素(BE)、二胺氧化酶(DAO)及D-乳酸(D-Lac)水平。结果 2组患者治疗前血清PCT、BE、DAO、D-Lac水平比较差异均无统计学意义(P>0.05),2组患者治疗3、7 d血清PCT、BE、DAO、D-Lac水平显著低于治疗前(P<0.05),2组患者治疗7 d血清PCT、BE、DAO、D-Lac水平显著低于治疗3 d(P<0.05);治疗3、7 d,观察组患者血清PCT、BE、DAO、D-Lac水平低于对照组(P<0.05)。结论 生长抑素可有效抑制老年脓毒症患者的炎症反应,提高肠黏膜屏障功能。
Abstract:
Objective To investigate the effect of somatostatin on intestinal mucosal barrier function in elderly patients with sepsis.Methods Thirty-six elderly patients with sepsis admitted to the Third Affiliated Hospital of Xinxiang Medical University from April 2017 to March 2018 were selected as the research objects,and they were divided into observation group and control group according to the treatment methods,with eighteen cases in each group.The patients in the control group were treated with routine treatment such as antibiotics,fluid resuscitation,inhibition of gastric acid secretion and low molecular weight heparin.On the basis of routine treatment,the patients in the observation group were treated with somatostatin 6 mg,dissolved in 60 mL saline,continuous intravenous drip for 24 hours for a week.The level of serum procalcitonin (PCT) were detected by colloidal immunochromatography,and the levels of serum bacterial endotoxin (BE),diamine oxidase (DAO) and D-lactate (D-Lac) were measured by dynamic turbidimetry before treatment and the 3ed and 7th day of treatment.Results There was no significant difference in serum PCT,BE,DAO and D-Lac levels between the two groups before treatment (P>0.05).The levels of serum PCT,BE,DAO and D-Lac on the 3rd and 7th day of treatment were significantly lower than those before treatment in the two groups (P<0.05).The levels of serum PCT,BE,DAO and D-Lac on the 7th day of treatment were significantly lower than those on the 3rd day of treatment in the two groups (P<0.05).On the 3rd and 7th day of treatment,the levels of serum PCT,BE,DAO and D-Lac in the observation group were lower than those in the control group (P<0.05).Conclusion Somatostatin can effectively inhibit the inflammatory response and improve the intestinal mucosal barrier function in elderly patients with sepsis.
