摘要:
目的 观察米力农治疗急性加重期慢性充血性心力衰竭(CCHF)的临床疗效及其对患者血清中心肌营养素-1(CT-1)、S100B和N末端脑钠肽前体(NT-proBNP)水平的影响。方法 将新乡医学院第二附属医院2015年2月至2017年2月收治的80例急性加重期CCHF患者分为对照组和观察组,每组40例。2组患者均给予吸氧、利尿剂、醛固酮拮抗剂、血管紧张素转化酶抑制剂、洋地黄、硝酸酯等常规治疗,在此基础上,观察组患者经静脉泵入米力农10 mg(0.5 μg·kg-1·min-1);2组患者均治疗10 d。分别于治疗前和治疗后采用彩色超声测2组患者左心室射血分数(LVEF),采用双抗体夹心酶联免疫吸附法检测血清中CT-1、S100B水平;采用免疫层析法检测血清中NT-proBNP水平;并比较2组患者的治疗效果。结果 观察组和对照组患者治疗后总有效率分别为87.5%(35/40)、65.0%(26/40),观察组患者治疗总有效率高于对照组(χ2=6.687,P<0.05)。治疗前2组患者LVEF比较差异无统计学意义(P>0.05);治疗后2组患者LVEF均高于治疗前(P<0.05);且治疗后观察组患者LVEF高于对照组(P<0.05)。治疗前2组患者血清中CT-1、S100B、NT-proBNP水平比较差异均无统计学意义(P>0.05);治疗后2组患者血清中CT-1、S100B、NT-proBNP水平均低于治疗前(P<0.05);治疗后观察组患者血清中CT-1、S100B、NT-proBNP水平低于对照组(P<0.05)。结论 米力农能够改善CCHF急性发作患者的心功能,使患者血清中CT-1、S100B及NT-proBNP水平降低。
Abstract:
Objective To observe the clinical efficacy of milrinone in treatment of patients with acute exacerbation of chronic congestive heart failure(CCHF) and its effect on the serum levels of cardiotrophin-1 (CT-1),S100B,and N terminal brain natriuretic peptide (NT-proBNP).Methods Eighty patients with acute exacerbation CCHF in the Second Affiliated Hospital of Xinxiang Medical University from Februray 2015 to Februray 2017 were selected and divided into the control group(n=40) and the observation group(n=40).All patients were given conventional treatment such as oxygen inhalation,diuretic,aldosterone antagonist,angiotensin converting enzyme inhibitor,digitalis,nitrate;based on this,the patients in the observation group were given milrinone 10 mg by venous pump for 10 days.The left ventricular ejection fraction (LVEF) of patients in the two groups were measured by color ultrasound before and after treatment;the serum levels of CT-1,S100B of patients in the two groups were measured by double antibody sandwich enzyme-linked immunosorbent assay;the serum levels of NT-proBNP of patients in the two groups were measured by immunochromatography.The therapeutic effect was compared between the two groups.Results The total effective rate of patients in the observation group and the control group was 87.5% (35/40) and 65.0% (26/40),respectively;the total effective rate of patients in the observation group was significantly higher than that in the control group(χ2=6.687,P<0.05).There was no statistic difference in the LVEF of patients between the two groups before treatment(P>0.05);after treatment,the LVEF of patients in the two groups was significantly higher than that before treatment(P<0.01);the LVEF of patients in the observation group was significantly higher than that in the control group after treatment(P<0.05).There was no statistic difference in the serum levels of CT-1,S100B,NT-proBNP of patients between the two groups before treatment(P>0.05);after treatment,the serum levels of CT-1,S100B,NT-proBNP of patients in the two groups were significantly lower than those before treatment(P<0.05);the serum levels of CT-1,S100B,NT-proBNP of patients in the observation group were significantly lower than those in the control group after treatment(P<0.05).Conclusion Milrinone can improve the cardiac function of patients with acute exacerbation of CCHF,and decrease the serum levels of CT-1,S100B and NT-proBNP.
