摘要:
目的 探讨免疫性血小板减少症(ITP)患儿外周血T淋巴细胞相关细胞因子水平的变化及临床意义。方法 选取2016年7月至2017年10月新乡医学院第一附属医院收治的新诊断ITP患儿40例为观察组,并选择同期门诊体检的健康儿童31例为对照组,采用流式细胞微球阵列技术检测2组受试者外周血中Th1细胞因子白细胞介素(IL)-2、干扰素-γ(IFN-γ)、肿瘤坏死因子(TNF)及Th2细胞因子IL-4、IL-6、IL-10和Th17细胞因子IL-17A水平,对2组患者治疗前后血清IL-2、IL-4、IL-6、IL-10、TNF、IFN-γ、IL-17A水平进行比较,并分析观察组患者外周血T淋巴细胞相关细胞因子与血小板计数的相关性以及各血清细胞因子之间的相关性。结果 观察组患者治疗前血清IL-2、IL-6、TNF、IFN-γ、IL-17A水平显著高于对照组受试者(P<0.05),IL-4、IL-10水平显著低于对照组受试者(P<0.05)。观察组患者治疗后血清IL-2、IL-6、TNF、IFN-γ、IL-17A水平较治疗前显著降低(P<0.05),IL-4、IL-10水平较治疗前显著升高(P<0.05)。观察组患者治疗后血清IL-2、IL-4、IL-6、IL-10、TNF、IFN-γ、IL-17A水平与对照组比较差异无统计学意义(P>0.05)。观察组患者血清各细胞因子水平与血小板计数无显著相关性(P>0.05)。观察组患者血清IL-2与IL-6水平呈正相关(r=0.382,P=0.00),IL-6与IFN-γ水平呈正相关(r=0.759,P=0.00),其他细胞因子之间无相关性(P>0.05)。结论 新诊断ITP患儿体内存多种T淋巴细胞细胞因子紊乱,以Th1、Th17细胞因子升高为主,提示ITP患儿体内存在Th1、Th17细胞优势。
Abstract:
Objective To explore the changes and clinical significance of T lymphocyte related cytokines levels in peripheral blood in children with immune thrombocytopenia (ITP).Methods Forty newly diagnosed ITP children in the First Affiliated Hospital of Xinxiang Medical University from July 2016 to October 2017 were selected as observation group;at the same period,31 healthy children were selected as control group.The levels of cytokines of Th1 including interleukin(IL) -2,interferon-γ(IFN-γ),tumor necrosis factor (TNF),Th2(IL-4,IL-6,IL-10) and Th17(IL-17A) in peripheral blood of all subjects were detected by cytometric bead array technology and were compared between the two groups.The correlation between the serum levels of cytokines and platelets,and the correlation between cytokines of children in the obsevation group were analysed.Results Before treatment,the serum levels of IL-2,IL-6,TNF,IFN-γ and IL-17A of children in observation group were significantly higher than those in the control group,but the serum levels of IL-4,IL-10 were significantly lower than those in the control group (P<0.05).In observation group,the serum levels of IL-2,IL-6,TNF,IFN-γ and IL-17A of children after treatment were significantly lower than those before treatment,the serum levels of IL-4,IL-10 after treatment were significantlyhigher than those before treatment (P<0.05).After treatment,there was no statistic difference in the serum levels of IL-2,IL-4,IL-6,IL-10,TNF,IFN-γ and IL-17A between the observation group and control group(P>0.05).There was no correlation between the serum levels of various cytokines and platelet in observation group(P>0.05).There was a positive correlation between the serum levels of IL-2 and IL-6 in the observation group(r=0.382,P<0.05);there was a positive correlation between the serum levels of IL-6 and IFN-γ in the observation group(r=0.759,P<0.05);there was no correlation between other cytokines(P>0.05).Conclusion There are many disorders of T lymphocyte related cytokines in the newly diagnosed ITP children,and the Th1 and Th17 were mainly increased,which suggesting that there are Th1 and Th17 advantages in children with ITP.
