摘要:
目的　探讨外固定架联合接骨板内固定治疗Young-Burgess APCⅢ、VS型骨盆骨折的临床疗效。方法　选择2013年2月至2015年12月河南医学高等专科学校附属医院采用后环接骨板内固定及前环外固定架固定术治疗的Young-Burgess APCⅢ、VS型骨盆骨折患者19例，采用Matta 评价标准及Majeed 评分系统判断患者最终治疗效果。结果　患者随访10～24个月，平均（18.0±1.2）个月，所有患者均正常骨性愈合，根据Matta标准进行疗效评分，优良率84.21%（16/19）；根据Majeed标准进行骨盆骨折功能恢复评分，优良率78.95%（15/19）；发生骨盆外固定针道松动1 例，骶髂关节疼痛1 例。骨折复位残留距离与功能恢复评分呈负相关（r=-0.992，P<0.05）。结论　外固定架联合接骨板内固定治疗Young-Burgess APCⅢ、VS型骨盆骨折，能恢复骨盆环的结构并维持其稳定，具有简单、实用、创伤小、临床疗效满意等优点。
Abstract:
Objective　To explore the clinical effect of external fixator combined with bone plate internal fixation in the treatment of type Ⅲ and VS Young-Burgess APC pelvic fractures.Methods　Nineteen patients with type Ⅲ and VS Young-Burgess APC pelvic fractures who were treated with external fixator and bone plate internal fixation in the Affiliated Hospital of Henan Medical College from February 2013 to December 2015 were selected.The effect was evaluated by Matta evaluation criteria and Majeed scoring system.Results　All patients were followed up for 10-24 months，the average follow-up time was (18.0±1.2) months.All patients had normal bony union，the fineness rate was 84.21%(16/19) according to the Matta standard，and the fineness rate of pelvic function was 78.95%(15/19) according to Majeed score.Among the patients，there were one case of pelvic external fixation loosening and one case of sacroiliac pain.The quality of fracture reduction was negatively correlated with functional recovery（r=-0.992，P<0.05）.Conclusion　External fixator combined with bone plate fixation for the treatment of type Ⅲ and VS Young-Burgess APC pelvic fractures can restore the structure of pelvic ring and maintain its stability.It has the advantages of simpleoperation，utility，small trauma，satisfactory clinical effect and so on.
