摘要:
目的　探讨妊娠糖尿病（GDM）患者血清中基质细胞衍生因子-1α（SDF-1α）和C反应蛋白（CRP）水平与胰岛素抵抗（IR）的相关性。方法　选择2012年1月至2015年5月咸宁市中心医院收治的GDM患者60例为GDM组，另选取口服葡萄糖耐量试验正常的健康孕妇60例为对照组，比较2组孕妇血清中SDF-1α、CRP、空腹胰岛素（FINS）、空腹血糖（FBG）、三酰甘油（TG）、总胆固醇（TC）、糖化血红蛋白A1c（HbA1c）水平，计算胰岛素抵抗指数（IRI）。结果　GDM组孕妇血清SDF-1α、CRP、FINS、FBG、HbA1c水平及IRI显著高于对照组（P<0.01），但2组孕妇血清TG、TC水平比较差异无统计学意义（P>0.05）。GDM患者血清SDF-1α与CRP水平呈正相关（P<0.01）。GDM患者血清SDF-1α、CRP水平均与FINS、FBG、HbA1c水平及IRI呈正相关（P<0.01，P<0.05），但与TG、TC水平无显著相关性（P>0.05）。结论　SDF-1α和CRP可能参与了GDM的发生和发展。
Abstract:
Objective　To investigate the relationship between the levels of serum stromal cell derived factor-1α(SDF-1α),C-reactive protein(CRP)and insulin resistance(IR)in patients with gestational diabetes mellitus(GDM).Methods　Sixty patients with GDM in Xianning Central Hospital from January 2012 to May 2015 were selected as GDM group,and another 60 cases of normal healthy pregnant women were selected as control group.The levels of serum SDF-1α,CRP,fasting insulin(FINS),fasting blood glucose(FBG),triglyeride(TG),total cholesterol(TC),glycosylated hemoglobin A1c(HbA1c)were compared between the two groups,and the insulin resistance index(IRI)was calculated.Results　The levels of serum SDF-1α,CRP,FINS,FBG,HbA1c and IRI in GDM group were significantly higher than those in control group(P<0.01).But there was no significant difference in the levels of TG and TC between the two groups(P>0.05).The serum SDF-1α level was positively correlated to CRP level in GDM patients(P<0.01).The levels of serum SDF-1α,CRP were positively correlated to FINS,FBG,HbA1c and IRI in GDM patients(P<0.01,P<0.05).But there was no significant correlation between serum SDF-1α,CRP and TG,TC(P>0.05).Conclusion　SDF-1 and CRP may be involved in the occurrence and development of GDM.
