摘要:
目的　探讨女性免疫性不孕症患者血清中抗精子抗体（AsAb）、抗心磷脂抗体（AcAb）和抗子宫内膜抗体（AEMAb）水平与炎性因子的相关性，为女性不孕症的诊疗提供参考。方法　选择2014年1月至2014年12月湖北省中医院收治的免疫性不孕症女性患者51例作为观察组，同时选取正常生育的健康女性70例作为对照组，采用酶联免疫吸附试验法检测血清AsAb、AcAb、AEMAb、肿瘤坏死因子-α（TNF-α）、白细胞介素-1β（IL-1β）、白细胞介素-6（IL-6）及白细胞介素-12（IL-12）水平，对2组受试者血清AsAb、AcAb、AEMAb、TNF-α、IL-1β、IL-6及IL-12水平进行比较，并分析血清AsAb、AcAb、AEMAb水平与炎性因子的相关性。结果　观察组患者血清AsAb、AcAb及AEMAb水平显著高于对照组（P<0.01）。对照组受试者血清AsAb、AcAb、AEMAb阳性率分别为5.71%（4/70）、2.86%（2/70）、4.28%（3/70），观察组患者血清AsAb、AcAb、AEMAb阳性率分别为37.25%（19/51）、21.57%（11/51）、31.37%（19/51），观察组患者血清AsAb、AcAb、AEMAb阳性率显著高于对照组（P<0.01）。观察组患者血清IL-6及IL-12水平显著高于对照组（P<0.01，P<0.05），而TNF-α水平明显低于对照组（P<0.01）。Pearson相关性分析结果显示，观察组患者血清AsAb与IL-6、IL-12呈正相关（P<0.05）；血清AcAb、AEMAb与IL-6呈正相关（P<0.05,P<0.01），与TNF-α呈负相关（P<0.05）。结论　女性免疫性不孕症患者血清AsAb、AcAb、AEMAb水平与TNF-α、IL-6及IL-12水平有相关性，AsAb、AcAb、AEMAb可能参与了免疫性不孕症的发病机制，对女性免疫性不孕症的诊断具有参考意义。
Abstract:
Objective　To investigate correlation between the expression of serum antisperm antibody(AsAb),anticardiolipin antibody(AcAb),antiendometrial antibody(AEMAb) and inflammatory factor in female patients with immune infertility,and to provide reference for the diagnosis and treatment of female infertility.Methods　Fifty-one female patients with immune infertility in Hubei Provincial Hospital of Traditional Chinese Medicine From January 2014 to December 2014 were selected as observation group;at the same time,seventy healthy women were selected as control group.The levels of serum AsAb,AcAb,AEMAb,tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6 (IL-6) and interleukin-12 (IL-12) were detected by enzyme-linked immunosorbent assay.The levels of serum AsAb,AcAb,AEMAb,TNF-α,IL-1β,IL-6 and IL-12 were compared between the two groups,and the correlation between the levels of serum AsAb,AcAb,AEMAb and inflammatory factor was analyzed.Results　The levels of serum AsAb,AcAb and AEMAb in observation group were significantly higher than those in control group(P<0.01).The positive rates of serum AsAb,AcAb and AEMAb in control group were 5.71% (4/70),2.86% (2/70) and 4.28% (3/70) respectively;the positive rates of AsAb,AcAb and AEMAb in observation group were 37.25% (19/51),21.57% (11/51) and 31.37% (19/51) respectively;the positive rates of AsAb,AcAb and AEMAb in observation group were significantly higher than those in control group (P<0.01).The levels of serum IL-6 and IL-12 in observation group were significantly higher than those in control group(P<0.01,P<0.05),the level of TNF-α in observation group was significantly lower than that in control group(P<0.01).Pearson correlation analysis showed that serum AsAb was positively correlated with IL-6 and IL-12(P<0.05),serum AcAb and AEMAb were positively correlated with IL-6(P<0.05,P<0.01),but serum AcAb and AEMAb were negatively correlated with TNF-αin observation group(P<0.05).Conclusion　The levels of serum AsAb,AcAb and AEMAb are correlated to the levels of serum TNF-α,IL-6 and IL-12 in female patients with immune infertility,AsAb,AcAb and AEMAb may be involved in the pathogenesis of immune infertility,which can provide the reference value for the diagnosis of female immune infertility.
