摘要:
目的 探讨慢性阻塞性肺疾病并发肺纤维化(COPD-PF)的临床特点。方法 对收治的56例慢性阻塞性肺疾病(COPD)患者和40例COPD-PF患者,均于入院当天行肺部高分辨率CT(HRCT)检查,并进行肺功能和血气分析检查,比较2组患者的用力肺活量占预计值百分比(FVC%pred)、第1秒用力呼气容积占预计值百分比(FEV1%pred)、FEV1/FVC值及一氧化碳弥散量占预计值百分比(DLCO%pred)和动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)。结果 COPD-PF组患者的FVC%pred显著低于单纯COPD组(P<0.05)。COPD组和COPD-PF组患者的FEV1%pred比较差异无统计学意义(P>0.05)。COPD-PF组患者的FEV1/FVC显著高于COPD组(P<0.05)。COPD-PF组患者的DLCO%pred较COPD组显著降低(P<0.05)。COPD-PF组患者的PaO2和PaCO2均显著低于COPD组(P<0.05)。56例COPD患者均出现肺气肿,其中38例患者出现肺大泡。40例COPD-PF患者均出现了肺气肿,同时胸膜下有蜂窝状影,其中6例患者呈现网络状以及毛玻璃样,5例患者出现肺大泡。结论 COPD-PF与COPD属于不同的2种疾病,掌握COPD-PF的临床特点有助于临床诊断和治疗。
Abstract:
Objective To study the clinical characteristics of chronic obstructive pulmonary disease (COPD)complicated with pulmonary fibrosis (PF)(COPD-PF).Methods Fifty-six patients with COPD and 40 patients with COPD-PF were selected.Lung was detected by high resolution computed tomography(HRCT),meanwhile,lung function and blood gas analysis were done on the day of admission.Forced vital capacity percentage of predicted value(FVC%pred),forced expiratory volume in one second percentage of predicted value(FEV1% pred),FEV1/FVC,diffusion capacity for carbon monoxide of lung percentage of predicted value(DLCO%pred),arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2) were compared between the two group.Results The level of FVC%pred was lower significantly in COPD-PF group than that in COPD group (P<0.05).The level of FEV1%pred was no significant difference between the two groups(P>0.05).The level of FEV1/FVC was higher significantly in COPD-PF group than that in COPD group (P<0.05).The level of DLCO% pred was lower significantly in COPD-PF group than that in COPD group (P<0.05).The level of PaO2 and PaCO2was lower significantly in COPD-PF group than that in COPD group (P<0.05).Emphysema was founded in all 56 patients with COPD and pneumatocele was founded in 38 patients with COPD.Emphysema and subpleural honeycomb shadow were founded in all 40 patients with COPD-PF,network-like and ground-glass appearance were founded in 6 patients and pneumatocele was founded in 5 patients with COPD-PF.Conclusion COPD-PF and COPD belong to different lesion category.To grasp the clinical features of COPD-PF is contribute to its clinical diagnose and treatment.
