摘要:
目的 分析伴中央颞区棘波的自限性癫痫(SeLECTS)患儿的主观睡眠状况及影响因素。
方法 选择2022年1月至2023年1月新乡医学院第一附属医院收治的67例SeLECTS患儿为病例组,选择同期来本院健康体检的151例健康儿童为对照组。应用儿童睡眠习惯问卷(CSHQ)评估2组儿童的睡眠质量,比较2组儿童睡眠质量不良发生率。根据CSHQ总分将病例组患儿分为睡眠质量良好组和睡眠质量不良组,比较2组儿童的性别、年龄、是否为首次发作、是否经过药物治疗、发作频率、非快速动眼(NREM)期放电指数,应用多因素logistic回归分析SeLECTS患儿睡眠质量不良的影响因素。
结果 病例组患儿CSHQ 总分显著高于对照组(P<0.05)。病例组患儿就寝习惯不良、睡眠焦虑、异态睡眠评分显著高于对照组(P<0.05);病例组与对照组儿童睡眠持续时间不规律、睡眠呼吸障碍、白天嗜睡、夜醒、入睡潜伏期延长评分比较差异无统计学意义(P>0.05)。对照组和病例组儿童睡眠质量不良比例分别为53.0% (80/151)、80.6% (54/67);病例组睡眠质量不良比例显著高于对照组(χ2=14.944,P<0.05)。睡眠质量良好组与睡眠质量不良组患儿的年龄和放电指数比较差异有统计学意义(P<0.05);睡眠质量良好组与睡眠质量不良组患儿的性别、是否服用抗癫痫药物、是否为首次发作、发作频率方面比较差异无统计学意义(P>0.05)。多因素logistic回归分析结果显示,NREM期放电指数增加是SeLECTS患儿出现睡眠质量不良的风险因素(比值比=25.297,95%置信区间4.583~139.634,P<0.05)。
结论 SeLECTS患儿较同年龄段儿童更容易存在睡眠问题,主要表现在睡眠焦虑、就寝习惯不良、异态睡眠3个层面。NREM期放电指数的增加是SeLECTS患儿出现睡眠质量不良的风险因素,NREM期放电指数越高的患儿越容易出睡眠质量不良。
Abstract:
Objective To analyze the subjective sleep status and influencing factors in children with self-limited epilepsy with centrotemporal spikes (SeLECTS).
Methods A total of 67 children with SeLECTS admitted to the First Affiliated Hospital of Xinxiang Medical University from January 2022 to January 2023 were selected as the research subjects in the case group.A total of 151 healthy children who came to our hospital for health examinations during the same period were selected in the control group.The sleep quality of children in the two groups was evaluated using the Children′s Sleep Habits Questionnaire (CSHQ),and the incidence of poor sleep quality was compared between the two groups.According to the total score of CSHQ,the children in the case group were divided into the good sleep quality group and the poor sleep quality group.The gender,age,whether it was the first episode,whether medication treatment was received,frequency of episodes,and non-rapid eye movement (NREM) discharge index of children in the two groups were compared.The influencing factors of poor sleep quality in children with SeLECTS were analyzed using multivariate logistic regression.
Results The total score of CSHQ of children in the case group was significantly higher than that in the control group (P<0.05).The scores of poor sleeping habits,sleep anxiety,and abnormal sleep in the case group were significantly higher than those in the control group (P<0.05),while there was no statistically significant difference in the scores of irregular sleep duration,sleep breathing disorders,daytime drowsiness,nocturnal awakening,and long sleep latency between the two groups (P>0.05).The proportions of children with poor sleep quality in the control group and the case group were 53.0% (80/151) and 80.6% (54/67),respectively.The proportion of children with poor sleep quality in the case group was significantly higher than that in the control group(χ2=14.944,P<0.05).There was a statistically significant difference in age and discharge index of children between the good sleep quality group and the poor sleep quality group (P<0.05),while there was no statistically significant difference in gender,whether antiepileptic drugs were taken,whether it was the first episode,and seizure frequency between the good sleep quality group and the poor sleep quality group (P>0.05).The results of multivariate logistic regression analysis showed that an increase in the NREM discharge index was a risk factor for poor sleep quality in children with SeLECTS (odds ratio=25.297,95% confidence interval:4.583-139.634,P<0.05).
Conclusion Children with SeLECTS are more prone to sleep problems than their peers,mainly manifested in sleep anxiety,poor sleeping habits,and abnormal sleep.The increase in the NREM discharge index is a risk factor for poor sleep quality in children with SeLECTS,and children with a higher NREM discharge index are more likely to suffer poor sleep quality.
