摘要:
目的 探讨医用射线抗护喷剂联合德莫林乳膏治疗乳腺癌患者术后放射性皮肤损伤的疗效。
方法 选择2019年1月至2021年6月焦作煤业(集团)有限责任公司中央医院收治的女性乳腺癌患者113例为研究对象,根据治疗方法将患者分为对照组(n=56)和观察组(n=57)。2组患者均接受放射治疗,对照组患者于放射治疗前1 d及放射治疗后将德莫林乳膏均匀涂抹于照射区域及区域外2 cm内,每日1次,连续使用2周。观察组在对照组患者治疗方案的基础上,于放射治疗前1 d及放射治疗后在患侧乳腺、腋窝、锁骨上区皮肤喷涂医用射线防护喷剂, 各部位早晚各喷涂1次,0.02 mL·cm-2,连续喷涂2周。药物治疗2周后,分别记录2组放射治疗≤2周和>2周患者急性放射性皮炎的发生情况,并计算总发生率。于放射治疗2周后,依据急性放射性损伤程度分级观察并评估患者皮肤损伤发生情况。记录2组患者发生Ⅱ级及以上皮肤损伤的累积剂量。
结果 药物治疗2周后,观察组患者的急性放射性皮炎发生率显著低于对照组(χ2=7.983,P<0.05)。放射治疗≤2周,观察组患者的急性放射性皮炎发生率显著低于对照组(χ2=5.423,P<0.05);放射治疗>2周,2组患者的急性放射性皮炎发生率比较差异无统计学意义(χ2=0.444,P>0.05)。药物治疗2周后,观察组患者的皮肤损伤程度分级显著优于对照组(Z=3.129,P<0.05)。对照组和观察组患者中发生Ⅱ级及以上皮肤损伤患者的累积剂量分别为(4 318.87±323.72)、(4 929.87±324.61)cGY;观察组患者中发生Ⅱ级及以上皮肤损伤患者的累积剂量显著高于对照组(P<0.05)。
结论 医用射线抗护喷剂联合德莫林乳膏应用于接受放射治疗的乳腺癌患者,能有效降低患者急性放射性皮炎发生率及皮肤损伤程度,提高发生Ⅱ级及以上放射性皮肤损伤的累积剂量。
Abstract:
Objective To investigate the effect of medical radiation protection spray combined with demolin cream in the treatment of radiation-induced skin injury in patients with breast cancer.
Methods A total of 113 female patients with breast cancer who were admitted to the Central Hospital of Jiaozuo Coal Industry (Group) Co.,Ltd.from January 2019 to June 2021 were selected as the study subjects.According to the treatment methods,the patients were divided into the control group (n=56) and the observation group (n=57).The patients in the two groups received radiation therapy.The patients in the control group were evenly applied with demolin cream to the irradiation area and within 2 cm outside the area at 1 day before radiation therapy and after radiation therapy, once a day, for 2 consecutive weeks.On the basis of the treatment plan for patients in the control group, the patients in the observation group sprayed medical radiation protection spray on the skin of the affected breast, armpit and supraclavicular area at 1 day before radiation therapy and after radiation therapy, once in the morning and evening, with 0.02 mL·cm-2, continuously for 2 weeks.After 2 weeks of drug treatment, the incidence of acute radiation dermatitis in patients with radiation therapy ≤2 weeks and>2 weeks in the both groups was recorded, and the total incidence rate was calculated.After two weeks of drug treatment,the incidence of acute radiation dermatitis of patients in the two groups was recorded and the incidence rate was calculated.After two weeks of radiation therapy,the occurrence of skin damage of patients were evaluated by the grading of acute radiation damage,and recorded the cumulative dose of grade Ⅱ and above skin damage of patients in two groups.
Results After two weeks of drug treatment,the incidence of acute radiation dermatitis of patients in the observation group was significantly lower than that in the control group (χ2=7.983,P<0.05).The incidence of acute radiation dermatitis of patients in the observation group was significantly lower than that in the control group after radiation therapy for less than or equal to two weeks(χ2=5.423,P<0.05);there was no significant difference in the incidence of acute radiation dermatitis of patients between the two groups treated with radiation therapy for more than two weeks (χ2=0.444,P>0.05).After two weeks of drug treatment,the degree of skin damage of patients in the observation group was significantly better than that in the control group (Z=3.129,P<0.05).The cumulative doses of patients with grade Ⅱ or above skin injury in the control group and observation group were (4 318.87 ± 323.72) and (4 929.87±324.61) cGY,respectively;the cumulative dose of patients with grade Ⅱ or above radiation dermatitis in the observation group was significantly higher than that in the control group (P<0.05).
Conclusion Medical radiation protection spray combined with demolin cream can effectively reduce the incidence of acute radiation dermatitis and the degree of skin damage of patients with breast cancer undergoing radiotherapy,and increase the accumulation of radiation dermatitis of grade II and above.
