摘要:
目的 探讨血小板内皮聚集受体1(PEAR1)基因单核苷酸多态性与癫痫持续状态后患者认知功能的关系。
方法 选择2020年1月至2022年1月安阳市人民医院收治的60例癫痫持续状态患者为研究对象(观察组)。另选择60例同期来院进行体检的健康志愿者为对照组。采用简易智力状态检查量表(MMSE)评估2组受试者的认知功能,并检测所有受试者PEAR1基因中rs12041331位点基因分型。根据MMSE评分结果将观察组患者分为认知功能正常组(n=40)和认知功能障碍组(n=20)。收集所有患者的临床资料,采用单因素和多因素logistic回归分析影响癫痫持续状态后患者认知功能的危险因素。
结果 观察组患者PEAR1基因rs12041331位点AA、GG、GA基因型分布频率与对照组比较差异有统计学意义(P<0.05)。观察组患者MMSE评分显著低于对照组(P<0.05)。单因素分析结果显示,认知功能障碍组患者的性别、年龄、发作类型、文化程度、服用抗癫痫药物种类、家族史与认知功能正常组比较差异无统计学意义(P>0.05);认知功能障碍组与认知功能正常组患者的PEAR1基因rs12041331位点基因分型、每个月发作频率、病程、起病年龄和MMSE评分比较差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,PEAR1基因rs12041331位点AA基因型、每个月发作频率>4次、病程时间长、起病年龄小、低MMSE评分是癫痫持续状态后患者发生认知功能障碍的危险因素(P<0.05)。
结论 PEAR1基因rs12041331位点AA基因型是癫痫持续状态后患者发生认知功能障碍的危险因素,携带AA基因型的癫痫持续状态患者更易发生认知功能障碍。
Abstract:
Objective To investigate the relationship between platelet endothelial aggregation receptor 1 (PEAR1) gene single nucleotide polymorphisms and cognitive function in patients after status epilepticus.
Methods Sixty patients with status epilepticus admitted to Anyang People′s Hospital from January 2020 to January 2022 were selected as the study subjects (observation group).Another 60 healthy volunteers who came to the hospital for physical examination during the same period were selected as the control group.The cognitive function of all subjects was assessed by using the mini-mental state examination scale (MMSE),and the genotype at rs12041331 locus in the PEAR1 gene of all subjects was detected.The patients in the observation group were divided into the normal cognitive function group(n=40) and the cognitive dysfunction group(n=20) according to the MMSE score.The clinical data of all patients were collected,and the risk factors affecting cognitive function in patients with status epilepticus were analyzed by using univariate and multivariate logistic regression.
Results The distribution frequencies of AA,GG and GA genotypes at rs12041331 locus of the PEAR1 gene in the observation group were significantly different from those in the control group (P<0.05).The MMSE score of patients in the observation group was significantly lower than that in the control group (P<0.05).Univariate analysis showed that there was no significant difference in gender,age,seizure type,education level,type of antiepileptic drugs taken and family history between the cognitive dysfunction group and the normal cognitive function group (P>0.05).There was significant difference in the genotype of rs12041331 locus of the PEAR1 gene,monthly seizure frequency,course of disease,onset age and MMSE score of patients between the cognitive dysfunction group and the normal cognitive function group (P<0.05).Multivariate logistic regression analysis showed that the AA genotype of rs12041331 locus in the PEAR1 gene,frequency of seizures > 4 times per month,long duration of disease,young onset age,low MMSE score were risk factors for cognitive dysfunction after status epilepticus (P<0.05).
Conclusion The AA genotype at rs12041331 locus of PEAR1 gene is a risk factor for cognitive dysfunction in patients with status epilepticus.Patients with status epilepticus who carry the AA genotype are more likely to develop cognitive dysfunction.
