摘要:
静脉血栓栓塞症(VTE)是围手术期的严重并发症。VTE早期发病隐匿,临床中漏诊率与误诊率较高,早期诊断并给予及时干预可降低病死率、致残率等。既往的实验室检查指标如凝血常规、D-二聚体等均为血栓形成后的筛查指标,无法反映血栓形成早期隐匿的止、凝血异常,缺乏特异性的血栓前状态的诊断价值。新型早期血栓分子标志物凝血酶-抗凝血酶复合物(TAT)、组织型纤溶酶原激活物-抑制剂1复合物(t-PAIC)、纤溶酶-α2纤溶酶抑制剂复合物(PIC)、血栓调节蛋白(TM)等可在一定程度上早期预警血栓前状态,对评估围手术期VTE的发生具有重要指导意义。本文就TAT、t-PAIC、PIC、TM等新型血栓分子标志物在围手术期VTE早期诊断中的作用研究进展作一综述。
Abstract:
Venous thromboembolism (VTE) is a serious complication in perioperative period.The early onset of VTE is hidden,and the rate of missed diagnosis and misdiagnosis is high in clinical practice.Early diagnosis and timely intervention can reduce the mortality and disability rate.Previous laboratory tests such as routine coagulation and D-dimer are screening indicators after thrombosis,unable to reflect hidden anticoagulant abnormalities in the early stage of thrombosis,and lacking specific diagnostic value of pre-thrombotic state.New early thrombotic molecular markers such as thrombin-antithrombin complex (TAT),plasmin-α2-plasmininhibitorcomplex(PIC),tissue plasminogenactivator-inhibitor 1 complex (t-PAIC) and thrombomodulin (TM) can provide early warning of prethrombotic state to a certain extent.It has important guiding significance to evaluate the occurrence of perioperative VTE.This article reviews the research progress of TAT,t-PAIC,PIC,TM and other novel thrombotic molecular markers in early diagnosis of perioperative VTE.
