摘要:
糖尿病肾病(DN)是糖尿病微血管并发症,主要表现为血糖升高、蛋白尿、肾功能下降,也是患者后期肾功能衰竭的主要原因。临床上主要通过控制血压和血糖水平来抑制DN的进展,但效果不佳。现有研究发现,内质网应激及其介导的细胞凋亡是引起DN的重要原因,蛋白激酶R样内质网激酶(PERK)/CCAAT增强子结合蛋白同源蛋白 (CHOP)/门冬氨酸特异性半胱氨酸蛋白酶-12(caspase-12)信号通路在其中发挥关键性作用,其中PERK是内质网应激启动的标志,CHOP是内质网应激由抗凋亡作用开始转为促凋亡作用的重要分子,caspase-12是内质网应激介导细胞凋亡的最终执行分子。本文就内质网应激及相关信号通路介导的细胞凋亡在DN发病机制中的作用进行综述。
Abstract:
Diabetic nephropathy (DN) is a microvascular complication of diabetes,mainly manifested as elevated blood sugar,proteinuria,and decreased renal function.It is also the main cause of late renal failure.Clinically,the progression of DN is mainly inhibited by controlling blood pressure and blood sugar levels in clinical work,but the effect is not good.Existing studies have found that endoplasmic reticulum stress (ERS) and it mediated apoptosis are important causes of DN,protein kinase R-like endoplasmic reticulum kinase (PERK)/CCAAT enhancer-binding protein homologous protein (CHOP)/cysteine-containing aspartate-specific proteases-12 (caspase-12) signaling pathway plays a key role in it.PERK is the marker of endoplasmic reticulum stress initiation,and the CHOP is an important molecule in the transition of endoplasmic reticulum stress from anti-apoptotic to pro-apoptotic.Caspase-12 is the ultimate executor of endoplasmic reticulum stress-mediated apoptosis.This paper reviews the role of endoplasmic reticulum stress and related signaling pathways mediated apoptosis in pathogenesis of DN.
