摘要:
目的 探讨妊娠期糖尿病(GDM)患者血清微RNA(miRNA)-372-3p和葡萄糖转运蛋白4(GLUT4)水平与胰岛素抵抗(IR)的关系。方法 选择2020年8月至2021年1月西南医科大学附属医院收治的42例GDM患者为GDM组,另选择同期42名正常妊娠妇女为对照组。采用全自动生物化学分析仪检测2组受试者血清中总胆固醇(TC)、三酰甘油(TG)水平,葡萄糖氧化酶法检测2组受试者空腹血糖(FPG)水平,放射免疫法检测2组受试者空腹胰岛素(FINS)水平,葡萄糖耐量试验检测2组受试者餐后2 h血糖(2 h BG)水平,计算稳态模式评估(HOMA)IR指数(HOMA-IR)和HOMA胰岛β细胞功能指数(HOMA-β)。采用实时荧光定量聚合酶链式反应法检测2组受试者血清中miR-372-3p相对表达量,采用酶联免疫吸附法检测2组受试者血清中GLUT4水平。采用Pearson相关分析GDM组患者血清中miR-372-3p相对表达量及GLUT4水平与代谢相关指标的相关性,多元线性回归分析GDM患者血清中miR-372-3p相对表达量及GLUT4水平与HOMA-IR的相关性。结果 GDM组患者血清中miR-372-3p相对表达量显著高于对照组,GLUT4水平显著低于对照组 (P<0.05)。GDM组患者FBG、FINS、2 h BG、TC、TG水平及HOMA-IR显著高于对照组,HOMA-β显著低于对照组 (P<0.05)。Pearson相关性分析结果显示,GDM组患者血清miR-372-3p相对表达量与FBG、FINS、2 h BG、TC、TG水平及HOMA-IR 呈正相关,与GLUT4水平及HOMA-β呈负相关(P<0.05);GLUT4水平与FBG、FINS、2 h BG、TC、TG水平及HOMA-IR呈负相关,与HOMA-β呈正相关(P<0.05)。多元线性回归分析结果显示,校正年龄、孕前体质量指数、孕周、孕次、产次等指标前后,血清中miR-372-3p相对表达量均与HOMA-IR呈独立正相关(P<0.05),GLUT4水平均与HOMA-IR呈独立负相关(P<0.05)。结论 GDM 患者血清中miR-372-3p相对表达量显著升高,GLUT4水平显著降低,miR-372-3p相对表达量及GLUT4水平与HOMA-IR独立相关,可作为GDM患者的重要监测指标。
Abstract:
Objective To investigate the relationship between the serum microRNA (miRNA)-372-3p and glucose transporter 4 (GLUT4) levels and insulin resistance (IR) in patients with gestational diabetes (GDM).Methods A total of 42 GDM patients admitted to the Affiliated Hospital of Southwest Medical University from August 2020 to January 2021 were selected as the GDM group,and 42 normal pregnant women during the same period were selected as the control group.The serum levels of total cholesterol (TC) and triacylglycerol (TG) were detected by automatic biochemical analyzer,the fasting blood glucose (FPG) level was detected by glucose oxidase method,and the level of fasting insulin (FINS) was detected by radioimmunoassay.The homeostasis model assessment insulin resistance index (HOMA-IR) and homeostasis model assessment beta-cell function index (HOMA- β) were calculated.The relative expression of miR-372-3p in serum of the subjects in the two groups was detected by real-time quantitative polymerase chain reaction,and the serum level of GLUT4 of the subjects in the two groups was detected by enzyme-linked immunosorbent assay.The correlation between the relative expression of miR-372-3p,the GLUT4 level in serum and the metabolic related indicators of patients in GDM group was analyzed by Pearson correlation.The relationship between the relative expression of miR-372-3p,the GLUT4 level in serum and the HOMA-IR of patients in GDM group was analyzed by multiple linear regression analysis.Results The relative expression of miR-372-3p of patients in the GDM group was significantly higher than that in the control group,and the serum level of GLUT4 was significantly lower than that in the control group (P<0.05).The FPG,FINS,2 h BG,TC,TG levels and the HOMA-IR of patients in the GDM group were significantly higher than those in the control group,and the HOMA-β was significantly lower than that in the control group (P<0.05).Pearson correlation analysis results showed that the relative expression of miR-372-3p of patients in the GDM group was positively correlated with FBG,FINS,2 h BG,TC,TG levels and HOMA-IR(P<0.05);and was negatively correlated with HOMA-β and GLUT4 levels (P<0.05).The serum level of GLUT4 of patients in the GDM group was negatively correlated with FBG,FINS,2 h BG,TC,TG levels and HOMA-IR (P<0.05);and was positively correlated with HOMA-β (P<0.05).Multiple linear regression analysis showed that the relative expression of miR-372-3p was independently positively correlated with HOMA-IR (P<0.05),and the serum level of GLUT4 was independently negatively correlated with HOMA-IR before and after adjusting for age,progestation body mass index,gestational weeks,times of pregnancy and times of delivery (P<0.05).Conclusion The relative expression of miR-372-3p in serum of patients with GDM is significantly increased,and the GLUT4 level is significantly decreased.The relative expression of miR-372-3p and the level of GLUT4 in serum are independently related to IR and they can be used as important monitoring indexes for patients with GDM.
