摘要:
目的 探讨多囊卵巢综合征(PCOS)患者血清磷酸化哺乳动物雷帕霉素靶蛋白(p-mTOR)表达水平与血清睾酮(T)水平及胰岛素抵抗(IR)的相关性及临床意义。方法 选择2019年1月至2020年7月于新乡医学院第一附属医院妇产科住院的80例PCOS患者为PCOS组,另选择同期于新乡医学院第一附属医院体检的卵巢功能正常的80例女性为对照组。收集2组受试者的一般资料,包括年龄、身高、体质量、腰围、臀围、收缩压、舒张压,并计算腰臀比及体质量指数(BMI);采集2组受试者静脉血,采用双抗体夹心酶联免疫吸附试验法检测血清p-mTOR、哺乳动物雷帕霉素靶蛋白(mTOR)水平,电化学发光免疫分析法检测血清黄体生成素(LH)、卵泡刺激素(FSH)、T、雌二醇(E2)、孕酮(P)、泌乳素(PRL)水平,葡萄糖氧化酶法检测空腹血糖(FPG)水平,电化学发光免疫分析法检测空腹胰岛素(FINS)水平,计算LH/FSH及胰岛素抵抗指数(HOMA-IR);采用t检验比较2组间各指标的差异,Pearson相关分析PCOS 患者血清p-mTOR水平与血清T水平、HOMA-IR及BMI的相关性。结果 PCOS组患者血清p-mTOR、LH、T水平及LH/FSH、BMI、HOMA-IR值显著高于对照组(P<0.05);PCOS组与对照组受试者的年龄、腰围、臀围、腰臀比、收缩压、舒张压、mTOR、FSH、E2、P、PRL及FPG比较差异无统计学意义(P>0.05);PCOS患者血清p-mTOR水平与血清T水平、HOMA-IR呈正相关(r2= 0.076、0.071,P<0.05),与BMI无相关性(r2=0.011,P>0.05)。结论 PCOS患者血清p-mTOR表达水平显著升高,且PCOS患者血清p-mTOR与血清T水平及IR呈明显正相关,p-mTOR、T和IR可能协同参与 PCOS 的发生和发展。
Abstract:
Objective To explore the correlation of the expression of serum phosphorylated mammal target of rapamycin (p-mTOR) with testosterone (T) level and insulin resistance (IR) of patients with polycystic ovary syndrome (PCOS) and its clinical significance.Methods Eighty patients with PCOS admitted to the Department of Gynecology,the First Affiliated Hospital of Xinxiang Medical University from January 2019 to July 2020 were selected as PCOS group,and eighty women with normal ovarian function who were examined in the First Affiliated Hospital of Xinxiang Medical University were selected as control group.The general data of the subjects in the two groups were collected,including age,height,body weight,waist circumference,hip circumference,systolic blood pressure,diastolic blood pressure,and the waist-to-hip ratio and body mass index (BMI) were calculated.Venous blood was collected from the subjects in the two groups,and the double antibody sandwich enzyme-linked immunosorbent assay was used to detect the serum p-mTOR and mammal target of rapamycin (mTOR) levels,the electricitychemiluminescence immunoassay was used to detect the levels of serum luteinizing hormone (LH),follicle-stimulating hormone (FSH),T,estradiol (E2),progesterone (P) and prolactin (PRL)the glucose oxidase method was used to detect the fasting plasma glucose (FPG) level,electrochemiluminescence immunoassay method was used to detect the fasting insulin (FINS) levelLH/FSH and homeostasis model assessment-insulin resistance (HOMA-IR) were calculatedthe t-test was used to compare the differences in the indicators between the two groups,and the Pearson correlation was used to analyze the correlation of serum p-mTOR level and serum T level,HOMA-IR,BMI in PCOS patients.Results The serum p-mTOR,LH,T levels and LH/FSH,BMI,HOMA-IR values of patients in the PCOS group were significantly higher than those in the control group (P<0.05)there was no significant difference in age,waist circumference,hip circumference,waist-to-hip ratio,systolic blood pressure,diastolic blood pressure,mTOR,FSH,E2,P,PRL and FPG of the subjects between the PCOS group and control group (P>0.05).The serum p-mTOR level of PCOS patients was positively correlated with serum T level and HOMA-IR (r2= 0.076,0.071P<0.05),and it had no correlation with BMI (r2=0.011,P>0.05).Conclusion The expression of p-mTOR in the serum of PCOS patients is significantly increased,the serum p-mTOR level in PCOS patients is positively correlated with the serum T level and IR,which suggests that p-mTOR,T and IR maybe participate in the occurrence and development of PCOS.
