摘要:
目的　探讨子宫内膜异位症（EM）患者病灶切除术后自我管理行为与生存质量的相关性及生存质量的影响因素。方法　选择2019年2月至9月上海交通大学医学院附属国际和平妇幼保健院收治的EM患者212例为调查对象进行问卷调查。每例患者需填写3种问卷调查表，包括EM自我管理量表、子宫内膜异位症患者生存质量量表（EHP-5）和一般资料调查表。采用Pearson相关性分析探讨EM患者术后自我管理行为与生存质量的相关性；采用Spearman相关性分析和多元逐步回归分析探讨影响EM患者术后生活质量的因素。结果　本研究共纳入200例有效调查对象。200例EM患者术后EM自我管理量表评分为（72.60±12.45）分，其中症状管理维度为（23.42±4.09）分，生活管理维度为（14.85±3.39）分，情绪管理维度为（17.48±3.80）分，信息管理维度为（16.86±4.16）分。200例EM患者术后EHP-5评分为（408.60±187.65）分，其中生存质量核心量表评分为（200.00±95.36）分，生存质量标准量表评分为（208.60±103.89）分。Pearson相关性分析结果显示，200例EM患者自我管理量表的总分及其症状管理维度、生活管理维度、情绪管理维度、信息管理维度评分与EHP-5的总分及其生存质量核心量表评分、生存质量标准量表评分呈负相关（P<0.05)。Spearman相关性分析结果显示，EM患者的睡眠质量、运动强度、月经周期、痛经持续时间、痛经疼痛评分、是否影响工作、自觉心理压力与术后生存质量有关（P<0.05）;EM患者的年龄、体质量指数、文化程度、婚姻状况、经期与术后生存质量无关（P>0.05）。多元逐步回归分析结果显示，痛经疼痛评分、自觉心理压力、是否影响工作、月经周期为影响EM患者术后生存质量的因素（P<0.05）。 结论　EM患者自我管理行为与生存质量呈显著正相关，患者的痛经疼痛程度、自觉心理压力、是否影响工作、月经周期是影响患者术后生存质量的主要因素。
Abstract:
Objective　To investigate the relationship between self-management behavior and quality of life of patients with endometriosis (EM) after focal resection and the influencing factors for quality of life.Methods　A total of 212 patients with EM admitted to the International Peace Maternity and Child Health Hospital,School of Medicine，Shanghai Jiao Tong University from February 2019 to September 2019 were selected as survey subjects to conduct a questionnaire survey.Each patient needed to fill in three questionnaires,including the endometriosis self-management scale,the endometriosis health profile-5 (EHP-5) and the general information questionnaire.The correlation between postoperative self-management and quality of life in EM patients was explored by Pearson correlation analysis;Spearman correlation analysis and multiple stepwise regression analysis were used to explore the factors affecting the quality of life of patients with EM.Results　A total of 200 valid survey subjects were included in this study.The postoperative EM self-management scale score of 200 cases of EM patients was (72.60±12.45) score,of which symptom management dimension was (23.42±4.09) score,life management dimension was (14.85±3.39) score,and emotional management dimension was (17.48±3.80) score,information management dimension (16.86±4.16) score.The EHP-5 score of 200 cases of EM patients postoperative was 408.60±187.65,of which the core quality of life scale score was 200.00±95.36,and the quality of life standard scale score was 208.60±103.89.The Pearson correlation analysis showed that the total score of self-management scale and its symptom management dimension,life management dimension,emotional management dimension,information management dimension score of the 200 cases of EM patients were negative correlated with the total score of EHP-5 and its quality of life scale score,quality of life standard scale score (P<0.05).The Spearman correlation analysis results showed that sleep quality,exercise intensity,menstrual cycle,dysmenorrhea pain score,whether it affects work,conscious psychological pressure were related to the quality of life of EM patients after surgery (P<0.05);the age,body mass index,education level,marital status,menstrual period were not related to the quality of life of EM patients after surgery (P>0.05).The multiple stepwise regression analysis showed that the dysmenorrhea pain score,conscious psychological pressure,whether it affects work and the menstrual cycle were the risk factors that affected the quality of life of EM patients after surgery (P<0.05).Conclusion　The self-management behavior of EM patients is significantly positively correlated with the quality of life.The degree of dysmenorrhea pain,conscious psychological pressure,whether it affects work and menstrual cycle are the main influencing factors that affect the quality of life of EM patients after surgery.
