摘要:
目的 探讨关节镜下倒打髓内钉手术治疗股骨干骨折(FSF)的临床效果。方法 选择2016年1月至2019年8月于河南省洛阳正骨医院接受倒打髓内钉手术治疗的50例FSF患者为研究对象,按照治疗方法将患者分为观察组和对照组,每组25例。对照组患者采用徒手倒打髓内钉手术,观察组患者采用关节镜下倒打髓内钉手术。记录患者手术时间、术中出血量、伤口愈合时间、住院时间、膝关节功能恢复情况及术后并发症;术后6个月,根据美国特种外科医院(HSS)评分判定患者临床疗效。结果 观察组患者手术时间显著长于对照组,伤口愈合时间和住院时间显著短于对照组,术中出血量显著少于对照组(P<0.05)。观察组患者自主直腿抬高时间及屈膝达25°、60°、120°的时间显著短于对照组(P<0.05)。观察组和对照组患者治疗效果优良率分别为100.0%(25/25)、96.0%(24/25),2组患者治疗效果优良率比较差异无统计学意义(χ2=3.213,P>0.05)。观察组和对照组患者术后并发症发生率分别为4.0%(1/25)、8.0%(2/25),观察组患者术后并发症发生率显著低于对照组(χ2=1.036,P<0.05)。结论 关节镜下倒打髓内钉治疗FSF创伤小,术后患者膝关节功能恢复快,且术后并发症发生率低。
Abstract:
Objective To investigate the clinical effect of arthroscopic retrograde intramedullary nailing in the treatment of femoral shaft fracture (FSF).Methods Fifty patients with FSF who underwent arthroscopic retrograde intramedullary nailing in Luoyang Orthopedic-Traumatological Hospital of Henan Province from January 2016 to August 2019 were selected as the research subjects,and the patients were divided into observation group and control group according to the treatment methods,with 25 cases in each group.The patients in the control group were treated with manual retrograde intramedullary nailing,and the patients in the observation group were treated with arthroscopic retrograde intramedullary nailing.The operation time,intraoperative bleeding,wound healing time,hospitalization time,knee function recovery and postoperative complications were recorded.Six months after operation,the clinical efficacy was determined according to the American hospital for special surgery (HSS) score.Results Compared with the control group,the operation time in the observation group was significantly longer,the wound healing time and hospitalization time were significantly shorter,and the intraoperative bleeding was significantly reduced (P<0.05).The time of independent straight leg raising and genuflex up to 25°,60° and 120° in the observation group was significantly shorter than that in the control group (P<0.05).The fineness rate in the observation group and the control group was 100.0% (25/25) and 96.0% (24/25),respectively.There was no significant difference in the fineness rate between the two groups(χ2=3.213,P>0.05).The incidence of postoperative complications in the observation group and the control group was 4.0% (1/25) and 8.0% (2/25),respectively.The incidence of postoperative complications in the observation group was significantly lower than that in the control group (χ2=1.036,P<0.05).Conclusion The arthroscopic retrograde intramedullary nailing in the treatment of FSF has the advantages of less trauma,rapid recovery of knee function and low incidence of postoperative complications.
