摘要:
目的 探讨多重耐药菌(MDRO)感染肺炎老年患者外周血中微RNA(miR)-127-5p水平与辅助性T细胞(Th)、调节性T细胞(Treg)及其细胞因子的相关性。方法 选择2018年3月至2019年11月河北燕达医院收治的MDRO肺炎所致肺炎老年患者57 例作为MDRO肺炎组,选择同期收治的老年普通肺炎患者50例作为肺炎组,另选择体检健康者50例作为对照组。实时荧光定量聚合酶链反应法检测所有受试者外周血中miR-127-5p表达水平,流式细胞术检测所有受试者外周血中Th和Treg细胞水平,酶联免疫吸附试验检测所有受试者血清中γ-干扰素(IFN-γ)、白细胞介素(IL)-4、IL-7及转化生长因子-β(TGF-β)水平。结果 MDRO肺炎组患者外周血中 miR-127-5p 相对表达量显著低于肺炎组和对照组(P<0.05),肺炎组患者外周血中 miR-127-5p 相对表达量显著低于对照组(P<0.05)。MDRO肺炎组患者外周血中Th1、Treg水平显著低于肺炎组和对照组,Th2、Th17水平显著高于肺炎组和对照组(P<0.05);肺炎组患者外周血中Th1、Treg水平显著低于对照组,Th2、Th17水平显著高于对照组(P<0.05)。 MDRO肺炎组患者血清中IFN-γ水平显著低于肺炎组和对照组,IL-4、IL-17及TGF-β水平显著高于肺炎组和对照组(P<0.05);肺炎组患者血清中IFN-γ水平显著低于对照组, IL-4、IL-17及TGF-β水平显著高于对照组(P<0.05)。MDRO肺炎组患者外周血中miR-127-5p相对表达量与外周血中Th1、Treg水平及血清中IFN-γ水平呈显著正相关(P<0.05),与外周血中Th2、Th17水平及血清中IL-4、IL-17、TGF-β水平呈显著负相关(P<0.05)。结论 MDRO感染肺炎老年患者外周血中miR-127-5p水平异常降低,且miR-127-5p表达水平与Th1、Treg细胞介导的免疫反应有关。
Abstract:
Objective To investigate the relationship between the levels of microRNA(miR)-127-5p in peripheral blood and helper T cell (Th),regulator T cell (Treg) and their cytokines in the elderly pneumonia patients infected by multidrug resistance organism(MDRO).Methods Fifty-seven elderly patients with pneumonia infected by MDRO in Hebei Yanda Hospital from March 2018 to November 2019 were selected as MDRO pneumonia group,and 50 elderly patients with common pneumonia in the same period were selected as pneumonia group,and 50 healthy subjects in the same period were selected as control group.The expression of miR-127-5p in peripheral blood of all subjects was detected by real-time fluorescence quantitative polymerase chain reaction,the levels of Th and Treg in peripheral blood of all subjects were detected by flow cytometry,and the serum levels of interferon-γ (IFN-γ),interleukin (IL)- 4,IL-7 and transforming growth factor-β(TGF-β)of all subjects were detected by enzyme-linked immunosorbent assay.Results The relative expression of miR-127-5p in peripheral blood of patients in the MDRO pneumonia group was significantly lower than that in the pneumonia group and control group (P<0.05),and the relative expression of miR-127-5p in peripheral blood of patients in the pneumonia group was significantly lower than that in the control group (P<0.05).The numbers of Th1,Treg of patients in the MDRO pneumonia group were significantly lower than those in the pneumonia group and control group (P<0.05),and the numbers of Th2,Th17 were significantly higher than that in the pneumonia group and control group (P<0.05).The numbers of Th1,Treg of patients in the pneumonia group were significantly lower than those in the control group (P<0.05),and the numbers of Th2 and Th17 cells were significantly higher than those in the control group (P<0.05).The level of serum IFN-γ of patients in MDRO group was significantly lower than that in the pneumonia group and control group (P<0.05),and the serum IL-4,IL-17,TGF-β levels of pationts in the MDRO pneumonia group were significantly higher than those in the pneumonia group and control group (P<0.05).The level of IFN-γ in serum of patients in the pneumonia group was significantly lower than that in the control group (P<0.05),and the serum IL-4,IL-17,TGF-β levels of pativents in the pneumonia group were significantly higher than those in the control group (P<0.05).The relative expression of miR-127-5p in peripheral blood of patients in the MDRO group was positively correlated with Th1,Treg and IFN-γ levels (P<0.05),and was negatively correlated with Th2,Th17 and IL-4,IL-17,TGF-β levels (P<0.05).Conclusion The level of miR-127-5p in the peripheral blood of the elderly patients with pneumonia infected by MDRO abnormally decreased,and the expression of miR-127-5p is related to Th1 and Treg mediated immune response.
