摘要:
目的 探讨高强度间歇训练对急性心肌梗死(AMI)患者经皮冠状动脉介入术(PCI)后心脏康复的影响。方法 选择2018年8月至2019年7月于河南(郑州)中汇心血管病医院接受PCI治疗的70例AMI患者为研究对象,根据患者PCI后心脏康复治疗方法将患者分为对照组(n=31)和观察组(n=39),对照组患者PCI后接受常规心脏康复运动治疗,观察组患者PCI后接受高强度间歇训练,2组患者均连续训练3个月。对2组患者康复训练前后心功能、心肺运动能力、生活质量及PCI后6个月内主要心脏不良事件(MACE)进行比较。结果 康复训练前2组患者左心室射血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)比较差异无统计学意义(P>0.05);2组患者康复训练后LVEF、LVEDD显著大于康复训练前,LVESD显著小于康复训练前(P<0.05);康复训练后,观察组患者LVEF、LVEDD显著大于对照组(P<0.05),但2组患者LVESD比较差异无统计学意义(P>0.05)。康复训练前2组患者6 min步行试验(6-MWT)、静息心率(HRrest)、最大耗氧量(VO2max)比较差异无统计学意义(P>0.05);2组患者康复训练后6-MWT、VO2max显著高于康复训练前,HRrest显著低于康复训练前(P<0.05);康复训练后,观察组患者6-MWT、VO2max显著高于对照组(P<0.05),但2组患者HRrest比较差异无统计学意义(P>0.05)。康复训练前2组患者健康状况调查简表(SF-36)各维度评分比较差异均无统计学意义(P>0.05),2组患者康复训练后SF-36各维度评分显著高于较康复训练前(P<0.05);康复训练后,观察组患者SF-36各维度评分显著高于对照组(P<0.05)。PCI后6个月内,观察组患者心律失常、心绞痛、心力衰竭发生率显著低于对照组(P<0.05),2组患者再梗死发生率比较差异无统计学意义(P>0.05)。结论 高强度间歇训练可有效改善PCI后AMI患者的心功能和心肺运动能力,提高术后生活质量,减少MACE的发生。
Abstract:
Objective To investigate the effect of high-intensity intermittent training on the cardiac rehabilitation in patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI).Methods Seventy patients with AMI who underwent PCI in Zhonghui Cardiovascular Hospital of Henan (Zhengzhou) from August 2018 to July 2019 were selected as the research subjects,and the patients were divided into control group (n=31) and observation group (n=39) according to the cardiac rehabilitation treatment methods after PCI.The patients in the control group received routine cardiac rehabilitation exercise therapy after PCI,while the patients in the observation group received high-intensity intermittent training after PCI.All patients in the two groups received continuous training for three months.The heart function,cardiopulmonary locomotory capacity,quality of life and the major adverse cardiac events (MACE) within six months after PCI were compared between the two groups before and after rehabilitation training.Results There was no significant difference in the left ventricular ejection fraction (LVEF),left ventricular end systolic diameter (LVESD) and left ventricular end diastolic diameter (LVEDD) between the two groups before rehabilitation training (P>0.05).The LVEF and LVEDD after rehabilitation training were significantly higher than those before rehabilitation training,and the LVESD after rehabilitation training was significantly lower than that before rehabilitation training in the two groups (P<0.05).After rehabilitation training,the LVEF and LVEDD in the observation group were significantly higher than those in the control group (P<0.05),but there was no significant difference in the LVESD between the two groups (P>0.05).There was no significant difference in the 6-minute walk test (6-MWT),resting heart rate (HRrest) and maximal oxygen consumption (VO2max) between the two groups before rehabilitation training (P>0.05).The 6-MWT and VO2max after rehabilitation training were significantly higher than those before rehabilitation training,and the HRrest after rehabilitation training was significantly lower than that before rehabilitation training in the two groups (P<0.05).After rehabilitation training,the 6-MWT and VO2max in the observation group were significantly higher than those in the control group (P<0.05),but there was no significant difference in the HRrest between the two groups (P>0.05).There was no significant difference in each dimension score of the MOS item short from health survey (SF-36) between the two groups before rehabilitation training (P>0.05).The scores of each dimension of SF-36 after rehabilitation training were significantly higher than those before rehabilitation training in the two groups (P<0.05).The scores of each dimension of SF-36 in the observation group were significantly higher than those in the control group after rehabilitation training (P<0.05).Within six months after PCI,the incidences of arrhythmia,angina pectoris and heart failure in the observation group were significantly lower than those in the control group (P<0.05),but there were no significant difference in the incidence of reinfarction between the two groups (P>0.05).Conclusion High-intensity intermittent training can effectively improve the heart function and cardiopulmonary locomotory capacity,improve the quality of life,and reduce the occurrence of MACE in AMI patients after PCI.
