摘要:
目的 探讨外周血细胞因子及T淋巴细胞水平与慢性阻塞性肺疾病(COPD)严重程度的相关性。方法  选择2019年1月至2020年2月郑州大学附属郑州中心医院呼吸内科收治的COPD患者87例为研究对象,按照病情严重程度将COPD患者分为A组(n=15)、B组(n=19)、C组(n=25)、D组(n=28);A组COPD患者病情最轻,D组COPD患者病情最重。另选取同期体检健康者20例作为对照组。采用肺功能检测仪检测各组受试者第1秒用力呼气容积与用力肺活量(FEV1 /FVC)比值及第1秒用力呼气容积占预计值百分比(FEV1%pred)。各组受试者于清晨空腹状态下采集外周静脉血,采用流式细胞仪检测各组受试者血清细胞因子白细胞介素(IL)-6、IL-10、肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)水平以及T淋巴细胞CD3+、CD4+、CD8+水平和CD4+/CD8+比值,采用Pearson分析其与COPD严重程度的相关性。结果 A、B、C、D组COPD患者FEV1 /FVC比值和FEV1%pred显著低于对照组(P<0.05);a、b、c组copd患者fev>1 /FVC比值和FEV1%pred显著高于D组(P<0.05),A、B组COPD患者FEV1 /FVC比值和FEV1%pred显著高于C 组(P<0.05),a组copd患者fev>1 /FVC比值和FEV1%pred显著高于B组 (P<0.05)。A、B、C、D组COPD患者IL-6、IL-10、TNF-α、IFN-γ水平显著高于对照组(P<0.05)。A、B、C组COPD患者IL-6、TNF-α、IFN-γ水平显著低于D组,IL-10水平显著高于D组(P<0.05)。c组copd患者il-6、inf-γ水平显著高于a、b组,c组copd患者tnf-α水平显著高于a组(>P<0.05)。对照组、a组和b组受试者cd3>+、CD4+及CD4+/CD8+比值比较差异均无统计学意义(P>0.05);C、D组COPD患者CD3+、CD4+及CD4+/CD8+比值均显著低于对照组(P<0.05)。d组患者cd3>+、CD4+及CD4+ /CD8+ 比值显著低于A、B组(P<0.05); c组患者cd3+、CD4+细胞水平显著低于A组(P<0.05);5组间cd8>+水平比较差异均无统计学意义(P>0.05)。B组和C组患者细胞因子IL-6、TNF-α、IFN-γ水平与FEV1%pred呈负相关(P<0.05);C组患者IL-10水平与FEV1%pred呈正相关(P<0.05);c组患者cd3>+、CD4+水平及CD4+ /CD8+比值与FEV1%pred呈正相关(P<0.05);D组患者CD3+细胞水平与FEV1%pred呈正相关(P<0.05) 。结论 细胞因子IL-6、IL-10、TNF-α、IFN-γ和淋巴细胞亚群水平与COPD严重程度密切相关,监测上述指标对于判断患者病情、指导治疗以及评估预后具有重要的临床意义。undefinedundefinedundefinedundefinedundefinedundefinedundefinedundefined/html>
Abstract:
Objective To investigate the correlations of cytokines and T lymphocyte levels in peripheral blood and the severity of chronic obstructive pulmonary disease (COPD).Methods  Eighty-seven patients with COPD admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University from January 2019 to February 2020 were selected as research objects,and according to the severity of COPD,they were divided into four experimental groups as group A(n=15),group B(n=19),group C(n=25)and group D(n=28);the severity of COPD in group A was lightest,the severity of COPD in group D was severest.Additionally,20 healthy subjects were selected as control group.Pulmonary function test was used to detect the ratio of forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) and FEV1 to predicted value(FEV1% pred) of the subjects in every group.Peripheral venous blood of all the subjects in the morning under empty stomach in every group was collected,flow cytometry was used to detect the levels of interleukin(IL)-6,IL-10,tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),CD3+,CD4+ and CD8+ in peripheral venous blood,the CD4+/CD8+ ratios was calculated.The correlations between the above factors and the severity of COPD were analyzed by using Pearson correlation method.Results The ratios of FEV1/FVC and FEV1%pred of COPD patients in group A,group B,group C and group D were significantly lower than those in the control group (P<0.05);the ratios of FEV1/FVC and FEV1%pred in group A,group B and group C were significantly higher than those in group D (P<0.05);the ratios of FEV1/FVC and FEV1%pred in group A and group B were significantly higher than those in group C (P<0.05);the ratios of FEV1/FVC and FEV1%pred in group A were significantly higher than those in group B (P<0.05).The levels of IL-6,IL-10,TNF-α and IFN-γ of COPD patients in group A,group B,group C and group D were significantly higher than those in the control group (P<0.05);the levels of IL-6,TNF-α and IFN-γ of COPD patients in group A,group B,group C was significantly lower than those in group D(P<0.05);the level of IL-10 in group A,group B and group C was significantly higher than that in group D (P<0.05);the levels of IL-6 and IFN-γ of COPD patients in group C were significantly higher than those in group A and group B (P<0.05);the level of TNF-α of COPD patients in group C was significantly higher than that in group A (P<0.05).There was no significant difference in CD3+,CD4+ levels and CD4+/CD8+ ratio among the control group,group A and group B (P>0.05).The CD3+,CD4+levels and CD4+/CD8+ratio of COPD patients in group C and group D were significantly lower than those in the control group (P<0.05).The CD3+,CD4+ levels and CD4+/CD8+ ratio of COPD patients in group D were significantly lower than those in group A and group B (P<0.05).The CD3+ and CD4+ levels of COPD patients in group C were significantly lower than those in group A (P<0.05).There was no significant difference in CD8+ level among the five groups (P>0.05).There were negative correlations of the IL-6,TNF-α,IFN-γ levels and the FEV1%pred of COPD patients in both group B and group C (P<0.05);the IL-10 level was positively correlated with the FEV1%pred ratio of COPD patients in group C (P<0.05);the CD3+,CD4+ levels and CD4+/CD8+ ratio were positively correlated with the FEV1%pred of COPD patients in group C (P<0.05);the CD3+ level was positively correlated with the FEV1%pred of COPD patients in group D(P<0.05).Conclusion Cytokines IL-6,IL-10,TNF-α,IFN-γ and T lymphocyte levels are closely correlated with the severity of COPD.Monitoring these factors is of great clinical significance in assessing patients′ COPD conditions,delivering medical treatment and evaluating their prognosis.
