摘要:
目的　探讨利拉鲁肽注射液联合前列地尔注射液治疗糖尿病肾病（DN）的临床效果。方法　选择2017年5月至2019年11月赤峰学院附属医院收治的110例DN患者为研究对象，根据治疗方法将患者分为观察组和对照组，每组55例。对照组患者给予前列地尔注射液治疗5个月，观察组患者给予利拉鲁肽注射液和前列地尔注射液联合治疗5个月。分别于治疗前及治疗5个月后检测患者低密度脂肪蛋白胆固醇（LDL-C）、总胆固醇（TC）、高密度脂蛋白胆固醇（HDL-C）、三酰甘油（TG）、空腹血糖（FBG）、糖化血红蛋白（HbA1C）、血肌酐（Scr）及血清基质金属蛋白酶-9（MMP-9）、超敏C反应蛋白（hs-CRP）水平，并进行24 h尿蛋白定量；治疗5个月后进行疗效判定，观察2组患者治疗期间不良反应发生情况。结果　治疗前2组患者LDL-C、TC、HDL-C及TG水平比较差异无统计学意义（P＞0.05）；与治疗前比较，2组患者治疗后LDL-C、TC、TG水平显著降低，HDL-C水平显著升高（P＜0.05）；治疗后，观察组患者LDL-C、TC、TG水平显著低于对照组，HDL-C水平显著高于对照组（P＜0.05）。治疗前2组患者HbA1c、FBG水平比较差异无统计学意义（P＞0.05），2组患者治疗后HbA1c、FBG水平显著低于治疗前（P＜0.05）；治疗后，观察组患者HbA1c、FBG水平显著低于对照组（P＜0.05）。治疗前2组患者Scr水平及24 h尿蛋白定量比较差异无统计学意义（P＞0.05），2组患者治疗后Scr水平及24 h尿蛋白定量显著低于治疗前（P＜0.05）；治疗后，观察组患者Scr水平及24 h尿蛋白定量显著低于对照组（P＜0.05）。治疗前2组患者血清MMP-9、hs-CRP水平比较差异无统计学意义（P＞0.05），2组患者治疗后血清MMP-9、hs-CRP水平显著低于治疗前（P＜0.05）；治疗后，观察组患者血清MMP-9、hs-CRP水平显著低于对照组（P＜0.05）。观察组和对照组患者治疗总有效率分别为89.09%（49/55）、74.55%（41/55），观察组患者总有效率显著高于对照组（χ2=3.911，P＜0.05）。观察组和对照组患者不良反应发生率分别为 9.09%（5/55）、12.73%（7/55），2组患者不良反应发生率比较差异无统计学意义（χ2=0.374，P＞0.05）。结论　利拉鲁肽注射液联合前列地尔注射液治疗DN疗效显著，可有效调控患者糖代谢和脂代谢，缓解炎症反应，改善肾功能，且安全性好。
Abstract:
Objective　To investigate the clinical effect of liraglutide injection combined with alprostadil injection in the treatment of diabetic nephropathy (DN).Methods　　A total of 110 patients with DN admitted to the Affiliated Hospital of Chifeng University from May 2017 to November 2019 were selected as the research subjects,and the patients were divided into observation group and control group according to the treatment methods,with 55 cases in each group.The patients in the control group were treated with alprostadil injection for five months,while the patients in the observation group were treated with liraglutide injection and alprostadil injection for five months.Before treatment and after five months of treatment,the levels of low density lipoprotein cholesterol (LDL-C),total cholesterol (TC),high density lipoprotein cholesterol (HDL-C),triglyceride (TG),fasting blood glucose (FBG),glycosylated hemoglobin (HbA1c) and serum creatinine (Scr),matrix metalloproteinase-9 (MMP-9),high sensitivity C-reactive protein (hs-CRP) were detected,and the 24-hour urinary protein quantification was detected.The curative effect was evaluated after five months of treatment.The adverse reactions of the patients in the two groups were observed during the treatment.Results　There was no significant difference in the levels of LDL-C,TC,HDL-C and TG between the two groups before treatment (P＞0.05).Compared with before treatment,the levels of LDL-C,TC and TG were significantly decreased,while the level of HDL-C was significantly increased after treatment in the two groups (P<0.05).After treatment,the levels of LDL-C,TC and TG in the observation group were significantly lower than those in the control group,and the level of HDL-C in the observation group was significantly higher than that in the control group (P<0.05).There was no significant difference in the levels of HbA1c and FBG between the two groups before treatment (P＞0.05).The levels of HbA1c and FBG after treatment were significantly lower than those before treatment in the two groups (P<0.05).The levels of HbA1c and FBG in the observation group were significantly lower than those in the control group after treatment (P<0.05).There was no significant difference in the levels of Scr and 24-hour urine protein quantification between the two groups before treatment (P＞0.05).The levels of Scr and 24-hour urine protein quantification after treatment were significantly lower than those before treatment in the two groups (P<0.05).The levels of Scr and 24-hour urine protein quantification in the observation group were significantly lower than those in the control group after treatment (P<0.05).There was no significant difference in the levels of serum MMP-9 and hs-CRP between the two groups before treatment (P＞0.05).The levels of serum MMP-9 and hs-CRP after treatment were significantly lower than those before treatment in the two groups (P<0.05).The levels of serum MMP-9 and hs-CRP in the observation group were significantly lower than those in the control group after treatment (P<0.05).The total effective rate in the observation group and the control group was 89.09% (49/55) and 74.55% (41/55),respectively.The total effective rate in the observation group was significantly higher than that in the control group (χ2=3.911,P<0.05).The incidence of adverse reactions in the observation group and the control group was 9.09% (5/55) and 12.73% (7/55),respectively.There was no significant difference in the incidence of adverse reactions between the two groups (χ2=0.374,P＞0.05).Conclusion　Liraglutide injection combined with alprostadil injection has significant effect in the treatment of DN,which can effectively regulate glycometabolism and lipid metabolism,alleviate inflammatory reaction,improve renal function,and has good safety.
