摘要:
目的 探讨血浆内皮细胞膜微粒(EMPs)、血清白细胞介素-6(IL-6)和细胞间黏附分子-1(ICAM-1)与急性冠状动脉综合征患者经皮冠状动脉介入术(PCI)后支架再狭窄的相关性。方法 回顾性分析2016年5月至2018年11月广东医科大学附属医院收治的200例急性冠状动脉综合征患者的临床资料,患者均顺利完成PCI手术。术后1 a对患者进行冠状动脉造影,根据PCI术后是否出现支架再狭窄分为再狭窄组(n=36)和非狭窄组(n=164),并对再狭窄组患者的狭窄程度进行分级。分别于PCI术前1 h及术后1 d、1周、1个月、6个月、1 a,采用酶联免疫吸附试验检测2组患者血清IL-6、ICAM-1水平;流式细胞仪检测2组患者血浆EMPs水平。分析血清IL-6、ICAM-1水平与血浆EMPs水平的相关性及血浆EMPs水平与急性冠状动脉综合征患者PCI术后支架再狭窄的相关性。结果 PCI术前及术后1 d,再狭窄组和非狭窄组患者血清IL-6、ICAM-1及血浆EMPs水平比较差异均无统计学意义(P>0.05);术后1周、1个月、6个月、1 a,再狭窄组患者血清IL-6、ICAM-1及血浆EMPs水平高于非狭窄组(P<0.05)。术后1 a,再狭窄组患者中血管狭窄A级31例(86.11%),B级3例(8.33%),O级2例(5.56%)。急性冠状动脉综合征患者PCI术后血清IL-6、ICAM-1水平均与血浆EMPs呈正相关(r=0.033、0.530,P<0.05)。急性冠状动脉综合征患者PCI术后血浆EMPs水平与支架再狭窄的发生呈正相关(r=0.658,P<0.05)。结论 急性冠状动脉综合征患者PCI术后血清IL-6、ICAM-1及血浆EMPs水平均升高,血浆EMPs水平与PCI术后支架再狭窄的发生有关。
Abstract:
Objective To investigate the correlation between the plasma endothelial microparticles (EMPs),serum interleukin-6 (IL-6) and intercellular adhesion molecule-1 (ICAM-1) and stent restenosis after percutaneous coronary intervention (PCI) of patients with acute coronary syndrome.Methods  The clinical data of 200 patients with acute coronary syndrome admitted to the Affiliated Hospital of Guangdong Medical University from May 2016 to November 2018 was retrospectively analyzed.All the patients successfully completed PCI.All patients were evaluated for coronary arteryangiography at 1 a after operation,and they were divided into restenosis group (n=36) and non-stenosis group (n=164) according to whether stent restenosis occurred after PCI,and the degree of restenosis of patients in the restenosis group was graded.The serum IL-6,ICAM-1 levels were detected at 1 h before PCI and 1 d,1 week,1 month,6 months,1 year after PCI.The plasma EMPs levels were measured by flow cytometry in both groups.The correlation between serum IL-6,ICAM-1 levels and plasma EMPs levels and the correlation between plasma EMPs levels and stent restenosis of patients with acute coronary syndrome after PCI were analyzed.Results There was no significant difference in serum IL-6,ICAM-1 and plasma EMPs levels of patients between the restenosis group and the non-stenotic group before and 1 d after surgery (P>0.05);the serum levels of IL-6,ICAM-1 and plasma EMPs levels of patients in the restenosis group were higher than those in the non-stenosis group at 1 week,1 month,6 months and 1 year after operation(P<0.05).In the restenosis group,31 cases (86.11%) were grade A,3 cases (8.33%) were grade B and 2 cases (5.56%) were grade O.The serum levels of IL-6 and ICAM-1 of patients with acute coronary syndrome after PCI were positively correlated with the level of EMPs (r=0.033,0.530;P<0.05).The plasma EMPs level of patients with acute coronary syndrome after PCI was positively correlated with the occurrence of stent restenosis (r=0.658,P<0.05).Conclusion The serum levels of IL-6,ICAM-1 and the plasma levels of EMPs in patients with acute coronary syndrome after PCI increase and the plasma levels of EMPs are positively correlated with the occurrence of stent restenosis.
