摘要:
目的 探讨早餐前后血脂对电化学发光免疫法(ECLIA)检测肿瘤标志物甲胎蛋白(AFP)效果的影响。方法 选择濮阳市安阳地区医院2018年6月至2019年12月收治的原发性肝细胞癌(HCC)住院患者65例为HCC组,另选择同期收治的血清三酰甘油(TG)和胆固醇(CHOL)同时增高的门诊及住院高脂血症患者90例为高脂血症组,选择同期体检健康人员90例为正常对照组。所有受试者于同一日早餐前及早餐后2 h采集静脉血4 mL,应用全自动电化学发光免疫分析仪检测血清AFP水平,全自动生物化学分析仪检测CHOL和TG水平。分析各组受试者早餐前后AFP、CHOL、TG水平的差异,以及各组受试者早餐前后CHOL、TG水平差值与AFP水平差值的相关性。结果 高脂血症组和HCC组受试者早餐前、早餐后2 h血清AFP水平均显著高于正常对照组(P<0.05),HCC组受试者早餐前、早餐后2 h血清AFP水平均显著高于高脂血症组(P<0.05)。高脂血症组受试者早餐前和早餐后2 h 血清CHOL、TG水平均显著高于正常对照组和HCC组(P<0.05);HCC组受试者早餐前和早餐后2 h 血清CHOL、TG水平与正常对照组比较差异无统计学意义(P>0.05)。正常对照组、高脂血症组和HCC组受试者早餐后2 h 血清AFP水平与早餐前比较差异均无统计学意义(P>0.05);正常对照组、高脂血症组和HCC组受试者早餐后2 h 血清CHOL、TG水平均显著高于早餐前(P<0.05)。Pearson相关性分析结果显示,HCC组、高脂血症组及正常对照组受试者早餐前后CHOL、TG水平差值与AFP水平差值均无相关性(r=0.036、0.151,0.087、0.074,0.049,0.105;P>0.05)。结论 早餐前后血脂对ECLIA检测血清AFP水平无影响。
Abstract:
Objective To explore the effect of serum lipids before and after breakfast on the effect of electrochemiluminescence immunoassay (ECLIA) on detecting tumor marker alpha-fetoprotein (AFP).Methods A total of 65 patients with hepatocellular carcinoma(HCC) admitted to Anyang District Hospital of Puyang City from June 2018 to December 2019 were selected as HCC group.In addition,90 cases of outpatient and inpatient hyperlipidemia patients with increased serum triacylglycerol (TG) and cholesterol (CHOL) were selected as the hyperlipidemia group.At the same time,90 cases of healthy person during physical examination were selected as normal control group.Before breakfast and 2 h after breakfast,4 mL of venous blood was collected from everybody of all the subjects on the same day.The serum AFP level was detected by an automatic electrochemiluminescence immunoassay analyzer,the CHOL and TG levels were detected by an automatic biochemical analyzer.The difference levels of AFP,CHOL and TG of the subjects before breakfast and 2 h after breakfast in each group were analyzed,as well as the correlation between the difference values of CHOL,TG levels and the difference value of AFP before and after breakfast in each group were analyzed.Results  Before breakfast and 2 h after breakfast,the serum AFP levels of the subjects in the hyperlipidemia group and HCC group were significantly higher than that in the normal control group (P<0.05).Before breakfast and 2 h after breakfast,the serum AFP level of the subjects in the HCC group was significantly higher than that in the hyperlipidemia group (P<0.05).Before breakfast and 2 h after breakfast,the serum CHOL and TG levels of the subjects in the hyperlipidemia group were significantly higher than those in the normal control group and HCC group (P<0.05);there was no statistically significant difference in the serum CHOL and TG levels of the subjects between the HCC group and normal control group (P>0.05).Two hours after breakfast,there was no statistically significant difference in the serum AFP levels of the subjects among the normal control group,hyperlipidemia group and HCC group (P>0.05);the serum CHOL and TG levels at 2 h after breakfast of the subjects in the normal control group,hyperlipidemia group and HCC group were significantly higher than those before breakfast (P<0.05). Pearson correlation analysis showed that there was no correlation between the difference levels of CHOL,TG before and after breakfast and the difference level of AFP of patients in the HCC group,the hyperlipidemia group and the normal control group (r=0.036,0.151,0.087,0.014,0.049,0.105;P>0.05).Conclusion Before and after breakfast,lipid factors have no effect on serum AFP levels detected by ECLIA.
