摘要:
目的 探讨早期糖尿病肾病患者血清分泌型卷曲相关蛋白1(SFRP1)的表达及其临床意义。方法 选择2016年5月至2019年5月新乡医学院第一附属医院收治的117例2型糖尿病患者为研究对象,其中未并发糖尿病肾病患者54例(糖尿病组),并发糖尿病肾病患者63例 (糖尿病肾病组);另选择同期体检健康者47例为正常对照组。分别采集3组受试者晨起空腹外周静脉血,检测血清SFRP1、超氧化物歧化酶(SOD)、转化生长因子-β1(TGF-β1)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)及肌酐水平,并计算胰岛素抵抗指数(HOMA-IR);留过夜8、24 h尿,分别检测尿白蛋白排泄率和尿白蛋白/肌酐比值;分析血清SFRP1水平与早期糖尿病肾病患者临床相关指标的相关性。结果 糖尿病组和糖尿病肾病组患者FPG、HbA1c、空腹胰岛素、HOMA-IR、TC、LDL-C、TG、肌酐、尿白蛋白排泄率、尿白蛋白/肌酐、TGF-β1水平高于正常对照组(P<0.05),HDL-C、SOD水平低于正常对照组(P<0.05)。糖尿病肾病组患者肌酐、尿白蛋白排泄率、尿白蛋白/肌酐、TGF-β1水平高于糖尿病组,SOD水平低于糖尿病组(P<0.05);糖尿病肾病组患者FPG、HbA1c、空腹胰岛素、HOMA-IR、TC、HDL-C、LDL-C、TG水平与糖尿病组比较差异无统计学意义(P>0.05)。 糖尿病组患者血清SFRP1水平与正常对照组比较差异无统计学意义(P>0.05);糖尿病肾病组患者血清SFRP1水平高于糖尿病组和正常对照组(P<0.05)。糖尿病肾病患者血清SFRP1水平与FPG、HbA1c、空腹胰岛素、HOMA-IR、TC、LDL-C、TG、肌酐、尿白蛋白排泄率、尿白蛋白/肌酐、TGF-β1水平呈正相关(r=0.553、0.601、0.532、0.638、0.599、0.405、0.542、0.467、0.573、0.592、0.589,P<0.05),与HDL-C、SOD水平呈负相关(r=-0.581、-0.417,P<0.05)。结论 早期糖尿病肾病患者血清SFRP1水平上调,其可能参与了2型糖尿病肾病的发生、发展过程。
Abstract:
Objective To investigate the expression of serum secretory frizzled-related protein 1(SFRP1) in patients with early diabetic nephropathy and its clinical significance.Methods A total of 117 patients with type 2 diabetes admitted to the First Affiliated Hospital of Xinxiang Medical University from May 2016 to May 2019 were selected as the research objects,including 54 patients without diabetic nephropathy (diabetes group) and 63 patients with diabetic nephropathy (diabetic nephropathy group).Another 47 healthy people in the same period were selected as the normal control group.Fasting peripheral venous blood of subjects in the three groups was collected in the morning.Serum SFRP1,superoxide dismutase(SOD),transforming growth factor-β1(TGF-β1),fasting plasma glucose(FPG),hemoglobin A1c(HbA1c),fasting insulin,total cholesterol (TC),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),triglyceride (TG) and creatinine were measured,and homeostasis model assessment of insulin resistance(HOMA-IR)was calculated.After blood samples were collected,urine samples were kept for 8 and 24 hours overnight to detect urinary albumin excretion rate and the ratio of urinary albumin/creatinine.The correlation between the serum SFRP1 level and clinical indexes in patients with early diabetic nephropathy was analyzed.Results The levels of FPG,HbA1c,fasting insulin,HOMA-IR,TC,LDL-C,TG,creatinine,urinary albumin excretion rate,the ratio of urinary albumin/creatinine,TGF-β1 in the diabetic group and diabetic nephropathy group were higher than those in the normal control group,while the levels of HDL-C and SOD were lower than those in the normal control group (P<0.05).The levels of creatinine,urinary albumin excretion rate,the ratio of urinary albumin/creatinine and TGF-β1 in the diabetic nephropathy group were higher than those in the diabetes group,while the SOD level was lower than that in the diabetes group (P<0.05).There was no significant difference in the levels of FPG,HbA1c,fasting insulin,HOMA-IR,TC,HDL-C,LDL-C and TG between the diabetic nephropathy group and the diabetes group (P>0.05).There was no significant difference in the serum SFRP1 level between the diabetes group and the normal control group (P>0.05).The serum SFRP1 level in the diabetic nephropathy group was higher than that in the diabetes group and normal control group (P<0.05).Serum SFRP1 level was positively correlated with FPG,HbA1c,fasting insulin,HOMA-IR,TC,LDL-C,TG,creatinine,urinary albumin excretion rate,the ratio of urinary albumin/creatinine,TGF-β1(r=0.553,0.601,0.532,0.638,0.599,0.405,0.542,0.467,0.573,0.592,0.589;P<0.05),but it was negatively correlated with HDL-C and SOD(r=-0.581,-0.417;P<0.05).Conclusion Serum SFRP1 is up-regulated in patients with early diabetic nephropathy,which may be involved in the occurrence and development of type 2 diabetic nephropathy.
