摘要:
目的 探讨血清生长分化因子11(GDF11)与慢性阻塞性肺疾病(COPD)患者肺功能、活动耐力及生活质量的关系。方法 选择2018年5月至2019年7月新乡医学院第一附属医院呼吸科一病区收治的COPD急性加重期稳定后4周复查患者60例为观察对象,将肺功能Ⅰ、Ⅱ级患者分为对照组,肺功能Ⅲ、Ⅳ级患者分为观察组,每组30例。采用酶联免疫吸附试验法检测血清GDF11水平;应用肺功能仪检测患者的肺功能指标,包括用力肺活量(FVC)、1 s用力呼气量(FEV1)、FEV1/FVC、1 s用力呼气量占预计值百分比(FEV1%pred);采用6 min步行距离(6MWD)评估患者运动耐力;使用改良版英国医学研究委员会呼吸问卷(mMRC)评估患者呼吸困难严重程度;使用COPD评估量表(CAT)评估患者的生活质量。比较2组患者血清GDF11水平、FVC、FEV1、FEV1/FVC、FEV1%pred、6MWD及CAT评分、mMRC评分,并分析血清GDF11水平与FVC、FEV1、FEV1/FVC、FEV1%pred、6MWD、CAT评分、mMRC评分的相关性。结果 观察组患者血清GDF11水平、FVC、FEV1、FEV1/FVC、FEV1%pred及6MWD均显著低于对照组(P<0.01),CAT评分和mMRC评分均显著高于对照组(P<0.01)。COPD患者血清GDF11水平与年龄、CAT评分及mMRC评分均呈负相关(r=-0.410、-0.491、-0.499,P<0.05),与FVC、FEV1、FEV1/FVC、FEV1%pred及6MWD均呈正相关(r=0.454、0.543、0.493、0.556、0.589,P<0.05)。结论 COPD患者血清GDF11水平与其肺功能、6MWD及CAT评分、mMRC评分相关,GDF11可能参与COPD发病机制及病情进展,血清GDF11水平降低可能影响其肺功能、运动耐力和生活质量,提高血清GDF11水平有望成为改善COPD患者肺功能及预后的新靶点。
Abstract:
Objective To explore the relationship between serum growth differentiation factor 11(GDF11) and lung function,activity endurance and quality of life in patients with chronic obstructive pulmonary disease(COPD).Methods sixty patients admitted to the First Ward of Department of Respiratory,the First Affiliated Hospital of Xinxiang Medical University from May 2018 to July 2019 were selected as observation subjects who were reexamined at the fourth week after the acute exacerbation period of COPD.The patients with pulmonary function grade I and II were divided into control group,and the patients with pulmonary function grade III and IV were divided into observation group,with 30 patients in each group.The level of serum GDF11 was detected by enzyme-linked immunosorbent assay;Pulmonary function instrument was used to detect the patient′s lung function indexes,including forced vital capacity(FVC),forced expiratory volume in one second(FEV1),FEV1/FVC,forced expiratory volume in one second as a percentage of the predicted value(FEV1%pred);The 6 min walking distance(6MWD) was used to evaluate the patient′s activity endurance;the modified British Medical Research Council Respiratory Questionnaire(mMRC) was used to assess the severity of dyspnea;the COPD assessment scale(CAT) was used to assess the patient′s quality of life.The serum GDF11 levels,FVC,FEV1,FEV1/FVC,FEV1%pred,6MWD,CAT score and mMRC score of patients between the 2 groups were compared.The correlation between serum GDF11 level and FVC,FEV1/FVC,FEV1%PRED,6MWD,CAT score and mMRC score was analyzed.Results The serum GDF11 levels,FVC,FEV1,FEV1/FVC,FEV1%pred and 6MWD in the observation group were significantly lower than those in the control group(P<0.01),the CAT score and mMRC score were significantly higher than those in the control group(P<0.01).Serum GDF11 level in COPD patients was negatively correlated with age,CAT score,and mMRC score(r=-0.410,-0.491,-0.499;P<0.05),and positively correlated with FVC,FEV1,FEV1/FVC,FEV1%pred,and 6MWD( r=0.454,0.543,0.493,0.556,0.589;P<0.05).Conclusion Serum GDF11 levels in patients with COPD are related to lung function,6MWD score,CAT score,and mMRC score.GDF11 may be involved in the pathogenesis of COPD and disease progression.The decrease of serum GDF11 level may affect the lung function,activity endurance and quality of life of patients with COPD.Increasing serum GDF11 level is expected to become a new target for improving lung function and prognosis of COPD patients.
