摘要:
目的 探讨血清可溶性细胞间黏附分子-1(sICAM-1)水平与急性脑梗死患者血管性认知障碍(VCI)的相关性。方法 选择2018年1月至2019年6月新乡医学院第一附属医院收治的204例首次发病急性脑梗死患者为研究对象,于脑梗死发病后3个月采用中文版简易智能精神状态量表(MMSE)进行测评,根据测评结果分为VCI组(n=105)和无VCI组(n=99)。患者均于入院次日清晨抽取空腹肘静脉血5 mL,采用酶联免疫吸附试验法检测血清sICAM-1水平;采用受试者工作特征(ROC)曲线评价血清sICAM-1水平预测VCI的敏感度、特异度和准确度,计算Youden指数,确定最佳截断值;采用Spearman相关分析血清sICAM-1水平与VCI患者MMSE评分的相关性。结果 VCI组患者血清sICAM-1水平显著高于无VCI组(P<0.01)。VCI组患者中,发生轻度认知障碍54例,中度认知障碍34例,重度认知障碍17例;轻度、中度、重度认知障碍患者血清sICAM-1水平比较差异有统计学意义(F=48.441,P<0.01),中度、重度认知障碍患者血清sICAM-1水平显著高于轻度认知障碍患者(P<0.01),重度认知障碍患者血清sICAM-1水平显著高于中度认知障碍患者(P<0.01)。sICAM-1诊断VCI的曲线下面积为0.895(95%可信区间为0.849~0.942,P<0.01),血清sICAM-1水平为307.59 μg·L-1时获得最大Youden指数为0.688,血清sICAM-1水平预测脑梗死患者VCI的敏感度为98.1%,特异度为70.7%。血清sICAM-1水平与VCI患者MMSE评分呈显著负相关(r=-0.646,P<0.01)。结论 急性脑梗死VCI患者血清sICAM-1水平升高,血清sICAM-1水平与VCI患者MMSE评分呈显著负相关,血清sICAM-1水平可在一定程度上预测急性脑梗死患者发生VCI的可能。
Abstract:
Objective To investigate the correlation between the level of serum soluble intercellular adhesion molecule-1 (sICAM-1) and vascular cognitive impairment (VCI) in patients with acute cerebral infarction.Methods Two hundred and four patients with the first onset acute cerebral infarction in the First Affiliated Hospital of Xinxiang Medical University from January 2018 to June 2019 were selected as the subjects.The patients were evaluated by Chinese version of minimum mental state examination (MMSE) at three months after onset of cerebral infarction.According to the evaluation results,the patients were divided into VCI group (n=105) and non-VCI group (n=99).Five milliliters of fasting elbow venous blood was taken from patients in the next morning after admission.The level of serum sICAM-1 was detected by enzyme-linked immunosorbent assay.The sensitivity,specificity and accuracy of the serum sICAM-1 level in predicting VCI were evaluated by receiver operating characteristic curve(ROC),and the Youden index was calculated to determine the best cut-off value.The correlation between the serum sICAM-1 level and MMSE score in patients with VCI was analyzed by spearman correlation.Results The level of serum sICAM-1 in the VCI group was significantly higher than that in the non-VCI group (P<0.01).In the VCI group,there were 54 cases of mild cognitive impairment,34 cases of moderate cognitive impairment and 17 cases of severe cognitive impairment.There was statistically significant difference in the level of serum sICAM-1 among the patients with mild,moderate and severe cognitive impairment (F=48.441,P<0.01).The level of serum sICAM-1 in the patients with moderate cognitive impairment and severe cognitive impairment was significantly higher than that in the patients with mild cognitive impairment,and the level of serum sICAM-1 in the patients with severe cognitive impairment was significantly higher than that in the patients with moderate cognitive impairment (P<0.01).The area under the curve of sICAM-1 in diagnosing VCI was 0.895 (95% confidence interval:0.84-0.942,P<0.01),the maximum Youden index was 0.688 when the level of serum sICAM-1 was 307.59 μg·L-1,and the sensitivity and specificity of the serum sICAM-1 level in predicting VCI of patients with cerebral infarction were 98.1% and 70.7%.The level of sICAM-1 was negatively correlated with MMSE score in the patients with VCI (r=-0.646,P<0.01).Conclusion The level of serum sICAM-1 increased in acute cerebral infarction patients with VCI.The level of serum sICAM-1 is negatively correlated with MMSE score in patients with VCI.The level of serum sICAM-1 can predict the possibility of VCI in patients with acute cerebral infarction to certain extent.
