摘要:
目的 了解河南省儿童医院2017~2019年血液肿瘤科住院患儿医院感染现状,为有效预防与控制医院感染提供科学依据。方法 通过横断面调查方法,调查河南省儿童医院血液肿瘤科2017年8月16日、2018年7月23日和2019年8月19日所有住院患儿的医院感染情况,并对调查资料进行比较分析。结果 2017~2019年血液肿瘤科住院患儿医院感染现患率分别为13.33%(16/120)、19.51%(24/123)、13.39%(17/127),2017~2019年医院感染现患率比较差异无统计学意义(χ2=2.385,P>0.05)。2017~2019年住院患儿医院感染最常见部位(58.33%),其次为血液系统(25.00%)、胃肠道(13.89%)和上呼吸道(2.78%)。2017~2019年血液肿瘤科医院感染患儿共检出病原菌9株,其中表皮葡萄球菌4株,大肠埃希菌2株,铜绿假单胞菌1株,无色杆1株,阴沟肠杆菌1株。2017~2019年血液肿瘤科患儿抗菌药物使用率分别为33.33%(40/120)、37.40%(46/123)、44.09%(56/127),治疗性使用抗菌药物占比分别为95.00%(38/40)、78.26%(36/46)、80.36%(45/56),一联使用抗菌药物占比分别为87.50%(35/40)、71.74%(33/46)、73.21%(41/56),治疗性用药患儿病原学标本送检率分别为71.05%(27/38)、72.22%(26/36)、77.78%(35/45); 2017~2019年血液肿瘤科患儿抗菌药物使用率、疗性使用抗菌药物占比、一联使用抗菌药物占比、治疗性用药患儿病原学标本送检率比较差异均无统计学意义(χ2=3.096、5.225、3.631、0.564,P>0.05)。结论 血液肿瘤科为医院感染防控重点科室,表皮葡萄球菌和大肠埃希菌为主要致病菌。应重点预防和控制下呼吸道感染,合理使用抗菌药物,降低医院感染发生率。
Abstract:
Objective To understand the current status of hospital infections in hospitalized children in the Department of Hematology and Oncology,Henan Children′s Hospital from 2017 to 2019,and so as to provide scientific basis for effective prevention and control of nosocomial infection.Methods A cross-sectional survey was used to investigate the nosocomial infection of all hospitalized children in the Department of Hematology and Oncology of Henan Children′s Hospital on August 16,2017,July 23,2018 and August 19,2019,and the data were compared and analyzed.Results  From 2017 to 2019,the prevalence rate of nosocomial infection among hospitalized children in Department of Hematologic Oncology was 13.33%(16/120),19.51%(24/123),and 13.39%(17/127),respectively.There was no significant difference in the prevalence rate of nosocomial infection among the three years(χ2 =2.385,P>0.05).From 2017 to 2019,the most common nosocomial infection sites were the lower respiratory tract (58.33%),followed by the blood system (25.00%),the gastrointestinal tract (13.89%) and the upper respiratory tract (2.78%).From 2017 to 2019,the most common nosocomial infection sites were the lower respiratory tract (58.33%),followed by the blood system (25.00%),the gastrointestinal tract (13.89%) and the upper respiratory tract (2.78%) ;a total of 9 strains of pathogenic bacteria were detected in children with nosocomial infections in the Department of Hematology Oncology,including 4 strains of Staphylococcus epidermoides,2 strains of Escherichia coli,1 strain of Pseudomonas aeruginosa,1 strain of Colorless stem and 1 strain of Enterobacter cloacae.From 2017 to 2019,the usage rate of antibiotics in children in the Department of Hematology Oncology was 33.33%(40/120),37.40%(46/123),44.09%(56/127),the proportion of therapeutic antibiotics was 95.00%(38/40),78.26%(36/46),and 80.36%(45/56),the proportion of using one antibiotic was 87.50%(35/40),71.74%(33/46),and 73.21%(41/56),the examination rate of pathogenic specimens among the children with therapeutic drugs was 71.05%(27/38),72.22%(26/36) and 77.78%(35/45),respectively.There was no statistically significant difference in the usage rate of antibiotics,the proportion of therapeutic antibiotics,the proportion of using one antibiotic,and the rate of examination of pathogenic specimens among the children with therapeutic drugs in the Department of Hematologic Oncology(χ2 =3.096,5.225,3.631,0.564;P>0.05).Conclusion Department of Hematologic Oncology is the key department of nosocomial infection prevention and control,and staphylococcus epidermidis and escherichia coli are the main pathogenic bacteria.Emphasis should be placed on the prevention and control of lower respiratory tract infection and rational use of antibiotics to reduce the incidence of nosocomial infection.
