摘要:
目的 探讨丁苯酞治疗大脑中动脉狭窄所致急性脑梗死的临床效果。方法 选择郑州大学附属郑州中心医院2017年8月至2019年8月收治的大脑中动脉狭窄所致急性脑梗死患者88例为研究对象,根据治疗方法分为观察组和对照组,每组44例。2组患者均给予常规治疗,在此基础上,对照组患者给予依达拉奉治疗,观察组患者在对照组治疗的基础上给予丁苯酞,连续治疗4周。 治疗前后检测2组患者血清C-反应蛋白(CRP)和白细胞介素-4(IL-4)水平,采用美国国立卫生研究院脑卒中量表(NIHSS)和Barthel指数评分量表评估患者神经功能缺损情况和日常生活活动能力,比较2组患者的治疗效果及不良反应发生情况。结果 治疗前2组患者CRP、IL-4水平及NIHSS、Barthel评分比较差异无统计学意义(P>0.05)。治疗后,2组患者CRP、IL-4水平均较治疗前下降(P<0.05);且观察组患者CRP、IL-4水平低于对照组(P<0.05)。与治疗前比较,治疗后2组患者NIHSS评分下降,Barthel评分升高(P<0.05)。与对照组比较,治疗后观察组患者NIHSS评分下降,Barthel评分升高(P<0.05)。对照组和观察组患者治疗总有效率分别为77.27%(34/44)和95.45%(42/44),观察组患者治疗总有效率高于对照组(χ2=6.175,P<0.05)。2组患者均未发生明显的过敏反应、胃肠道反应和出血倾向。结论 大脑中动脉狭窄所致急性脑梗死患者加用丁苯酞治疗更有助于改善炎症因子水平和神经功能,效果显著,安全性高。
Abstract:
Objective To explore the therapeutic effect of butylphthalide in treatment of acute cerebral infarction caused by middle cerebral artery stenosis.Methods Eighty-eight patients with acute cerebral infarction caused by middle cerebral artery stenosis admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University from August 2017 to August 2019 were selected as observation subjects.According to the treatment method,they were divided into observation group and control group,with 44 cases in each group.All patients were given routine therpy,based on this,the patients in the control group were given edaravone treatment,and the patients in the observation group were given butylphthalide on the basis of treatment of the control group;the patients in the two groups were treated for 4 consecutive weeks.The C-reactive protein (CRP) and interleukin-4 (IL-4) levels were detected before and after treatment.The nerve functional defects and daily living activities of the patients in the two groups were evaluated by the national institutes of health stroke scale (NIHSS) and Barthel index score scale,respectively.The treatment effects and adverse reactions of patients in the two groups were compared.Results There was no significant difference in the CRP,IL-4 levels and NIHSS,Barthel scores between the two groups before treatment (P>0.05).After treatment,the CRP and IL-4 levels in the two groups were lower than those before treatment (P<0.05);the levels of CRP and IL-4 in the observation group were lower than those in the control group (P<0.05).The NIHSS scores in the two groups after treatment were lower,while the Barthel scores were higher than those before treatment (P<0.05).Compared with the control group,the NIHSS scores in the observation group were lower and the Barthel scores were higher after treatment(P<0.05).The total effective rate of patients in the control group and the observation group was 77.27% (34/44) and 95.45% (42/44),respectively.The total effective rate of patients in the observation group was higher than that in the control group (χ2 =6.175,P<0.05).There was no significant allergic reaction,gastrointestinal reaction or bleeding tendency in both groups.Conclusion Butylphthalide can improve the level of inflammatory factors and nerve function in patients with acute cerebral infarction caused by middle cerebral artery stenosis with significant effect and high safety.
