摘要:
目的 探讨纤维支气管镜微波治疗对中心气道狭窄患者肺功能及生活质量的影响,为中心气道狭窄患者的临床治疗提供参考。方法 选择2014年10月至2015年12月许昌市中心医院收治的中心气道狭窄患者80例为研究对象,所有患者给予经纤维支气管镜微波治疗。分别于治疗前和治疗后,检测患者血气指标,包括血氧分压(PaO2)、血氧饱和度(SaO2)、二氧化碳分压(PaCO2)、酸碱度(pH)值;采用肺功能仪检测患者肺功能,包括一秒用力呼气量(FEV1)、用力肺活量(FVC)、最大呼气流量(PEF)、每分钟最大通气量(MVV);根据美国胸科协会气促分级标准评定患者气促情况,采用Karnofsky评分评估患者生活质量。治疗后评定患者临床疗效。结果 与治疗前比较,治疗后患者PaO2、SaO2升高,PaCO2降低(P<0.05);患者治疗前后pH值比较差异无统计学意义(P>0.05)。治疗后患者FEV1、FVC、PEF、MVV均高于治疗前,气促症状显著改善(P<0.05)。80例患者中,治疗显效32例(40.00%),有效38例(47.50%),无效(12.50%)10例,总有效率为87.50%(70/80)。患者治疗前后生活质量评分分别为(65.23±6.56)、(72.56±6.89)分,治疗后患者生活质量评分高于治疗前(P<0.05)。结论 纤维支气管镜微波治疗中心气道狭窄疗效显著,可有效缓解患者气促症状,改善患者肺功能,提高患者生活质量。
Abstract:
Objective To investigate the effect of fibrobronchoscopic microwave therapy on pulmonary function and quality of life in patients with central airway stenosis,and to provide reference for clinical treatment of patients with central airway stenosis.Methods A total of 80 patients with central airway stenosis admitted to Xuchang Central Hospital from October 2014 to December 2015 were selected as the study objects.All patients were treated with fibrobronchoscopic microwave therapy.Before and after treatment,the blood gas analysis indexes of patients were measured,including partial pressure of oxygen (PaO2),arterial oxygen saturation (SaO2),Partial pressure of carbon dioxide (PaCO2) and the value of potential of hydrogen (pH);the pulmonary function was measured by pulmonary function instrument,including forced expiratory volume (FEV1),forced vital capacity (FVC),peak expiratory flow (PEF) and maximum ventilation volume (MVV);the short breath of patients was evaluated according to the classification standard of short breath of the American Thoracic Society;and the quality of life was evaluated by Karnofsky score.The clinical effect was evaluated after treatment.Results Compared with before treatment,the PaO2 and SaO2 increased,and the PaCO2 decreased after treatment (P<0.05).There was no significant difference in pH value before and after treatment (P>0.05).After treatment,the FEV1,FVC,PEF and MVV were higher than those before treatment,and the symptom of short breath was significantly improved (P<0.05).Among the 80 patients,32 (40.00%) cases were excellence,38 (47.50%) cases were effective,and 10 (12.50%) cases were ineffective;the total effective rate was 87.50% (70/80).The score of quality of life of the patients were 65.23±6.56 and 72.56±6.89 before and after treatment.The score of quality of life after treatment was higher than that before treatment (P<0.05).Conclusions The fibrobronchoscopic microwave therapy is effective in the treatment of central airway stenosis,it can effectively relieve the symptoms of short breath,and improve the lung function and the quality of life of patients.
