目的 探讨锁骨骨折合并肋骨骨折行同期内固定术的临床效果。方法 选择2016年12月至2018年2月首都医科大学密云教学医院收治的48例锁骨骨折合并肋骨骨折患者为研究对象，按照手术方案分为观察组和对照组，每组24例。对照组患者进行分期内固定术治疗，观察组患者进行同期内固定术治疗。比较2组患者手术时间、麻醉时间、术中出血量、住院时间及住院费用；术后6个月评定2组患者临床疗效，观察2组患者术后并发症发生情况。结果 观察组患者手术时间、麻醉时间及住院时间显著短于对照组，术中出血量及住院费用显著少于对照组（P<0.05）。观察组和对照组患者总有效率分别为95.83%（23/24）、91.67%（22/24），2组患者总有效率比较差异无统计学意义（χ2=0.355，P>0.05）。观察组和对照组患者术后并发症发生率分别为12.5%（3/24）、16.7%（4/24），2组患者术后并发症发生率比较差异无统计学意义（χ2=0.167，P>0.05）。结论 锁骨骨折合并肋骨骨折患者行同期内固定术是可行的，可以显著缩短手术时间、麻醉时间及住院时间，减少术中出血量和住院费用。

Objective Objective To investigate the clinical effect of simultaneous internal fixation for clavicular fracture combined with rib fracture.Methods Forty-eight patients with clavicular fracture and rib fracture admitted to Miyun teaching hospital of Capital Medical University from December 2016 to February 2018 were selected as the study object.The patients were divided into observation group and control group according to observation group and control group,with 24 cases in each group.The patients in the control group were treated with staged internal fixation,while the patients in the observation group were treated with simultaneous internal fixation.The operation time,anesthesia time,intraoperative bleeding,hospitalization time and hospitalization cost were compared between the two groups.The clinical effect of patients in the two groups were evaluated at six months after operation,and the postoperative complications of patients in the two groups were observed.Results The operation time,anesthesia time and hospitalization time of the patients in the observation group were significantly shorter than those in the control group,and the intraoperative bleeding and hospitalization cost in the observation group were significantly lower than those in the control group (P<0.05).The total effective rate in the observation group and control group was 95.83% (23/24) and 91.67% (22/24),respectively.There was no significant difference in the total effective rate between the two groups (χ2=0.355,P>0.05).The incidence of postoperative complication in the observation group and control group was 12.5% (3/24) and 16.7% (4/24),respectively.There was no significant difference in the incidence of postoperative complication between the two groups (χ2=0.167,P>0.05).Conclusions Simultaneous internal fixation is feasible for patients with clavicular fracture combined with rib fracture,which can significantly shorten the operation time,anesthesia time and hospitalization time,reduce the intraoperative bleeding and hospitalization costs.
