摘要:
目的 探讨小剂量罗红霉素治疗支气管哮喘的临床效果及其对患儿细胞免疫功能、炎症因子水平的影响。方法 选择2017年1月至2018年1月南阳市中心医院收治的56例支气管哮喘患儿为研究对象,根据治疗方法将患儿分为观察组和对照组,每组28例。对照组患儿根据全球支气管哮喘防治创议给予常规治疗,观察组患儿在常规治疗基础上给予小剂量罗红霉素(2.5 mg·kg-1),空腹口服,每日2次,14 d为1个疗程。分别于治疗前后检测患儿肺功能,包括呼气峰值流速(PEF)、第1秒用力呼气量(FEV1)、FEV1/用力肺活量(FVC),并检测患儿T淋巴细胞亚群CD3+、CD4+、CD8+、 CD4+/CD8+水平及血清γ-干扰素(IFN-γ)、白细胞介素-8(IL-8)、白细胞介素-13(IL-13)水平;治疗后依据儿童支气管哮喘控制测试量表评估患儿临床疗效。结果 观察组患儿治疗显效率和总有效率分别为82.14%(23/28)、96.43%(27/28),对照组患儿治疗显效率和总有效率分别为42.86%(12/28)、78.57%(22/28),观察组患儿显效率和总有效率高于对照组(χ2=6.783、4.082,P<0.05)。治疗前2组患儿PEF、FEV1、FEV1/FVC比较差异均无统计学意义(P>0.05),2组患儿治疗后PEF、FEV1、FEV1/FVC显著高于治疗前(P<0.05);治疗后,观察组患儿PEF、FEV1、FEV1/FVC高于对照组(P<0.05)。治疗前2组患儿CD3+、CD4+、CD8+、CD4+/ CD8+水平比较差异均无统计学意义(P>0.05);与治疗前比较,治疗后2组患儿CD3+、CD4+、CD4+/ CD8+水平升高,CD8+水平降低(P<0.05);治疗后,观察组患儿CD3+、CD4+、CD4+/ CD8+水平高于对照组,CD8+水平低于对照组(P<0.05)。治疗前2组患儿IL-8、IL-13、IFN-γ水平比较差异均无统计学意义(P>0.05);与治疗前比较,治疗后2组患儿IL-8、IL-13水平降低,IFN-γ水平升高(P<0.05);治疗后,观察组患儿IL-8、IL-13水平低于对照组,IFN-γ水平高于对照组(P<0.05)。结论 小剂量罗红霉素能够增强支气管哮喘患儿细胞免疫功能,抑制炎症反应,改善肺功能,提高治疗效果。
Abstract:
Objective To investigate the clinical effect of low-dose roxithromycin in the treatment of bronchial asthma and its effect on cellular immune function and inflammatory factor level in children.Methods Fifty-six children with bronchial asthma admitted to Nanyang Central Hospital from January 2017 to January 2018 were selected as the study objects.The children were divided into observation group and control group according to the treatment methods,with 28 cases in each group.The children in the control group were given conventional treatment according to the global initiative for asthma.On the basis of routine treatment,the patients in the observation group were given low-dose roxithromycin (2.5 mg·kg-1) by fasting oral ,twice a day for 14 days.Before and after treatment,the pulmonary function was measured,including peak expiratory flow (PEF),forced expiratory volume in one second (FEV1) and the ratio of FEV1 to forced vital capacity(FVC)(FEV1/FVC);and the levels of T lymphocyte subsets CD3+,CD4+,CD8+,CD4+/CD8+ and the levels of serum interferon-γ(IFN-γ),interleukin-8(IL-8) and interleukin-13(IL-13) were measured.The clinical effect was evaluated by children asthma control test after treatment.Results The excellence rate and total effective rate were 82.14% (23/28) and 96.43% (27/28) in the observation group,them were 42.86% (12/28) and 78.57% (22/28) in the control group,respectively.The excellence rate and total effective rate in the observation group were higher than those in the control group (χ2=6.783,4.082;P<0.05).There was no significant difference in PEF,FEV1 and FEV1/FVC between the two groups before treatment (P>0.05).The PEF,FEV1 and FEV1/FVC after treatment were significantly higher than those before treatment in the two groups (P<0.05).The PEF,FEV1 and FEV1/FVC in the observation group were higher than those in the control group after treatment (P<0.05).There was no significant difference in the levels of CD3+,CD4+,CD8+,CD4+/CD8+ between the two groups before treatment (P>0.05).Compared with before treatment,the levels of CD3+,CD4+,CD4+/CD8+ increased,while the levels of CD8+ decreased after treatment in the two groups (P<0.05).After treatment,the level of CD3+,CD4+,CD4+/CD8+ in the observation group was higher than that in the control group,and the level of CD8+ in the observation group was lower than that in the control group (P<0.05).There was no significant difference in the levels of IL-8,IL-13 and IFN-γ between the two groups before treatment (P>0.05).Compared with before treatment,the levels of IL-8 and IL-13 decreased and the level of IFN-γ increased in the two groups after treatment (P<0.05).After treatment,the levels of IL-8 and IL-13 in the observation group was lower than those in the control group,and the level of IFN-γ in the observation group was higher than that in the control group (P<0.05).Conclusions Low-dose roxithromycin can enhance the cellular immune function,inhibit the inflammatory reaction,improve the lung function and improve the therapeutic effect in children with bronchial asthma.
