摘要:
目的 探讨血清脂联素(APN)、C1q/肿瘤坏死因子相关蛋白9(CTRP9)水平与冠状动脉病变程度的关系。方法 选择2018年3~9月在新乡医学院第一附属医院接受冠状动脉造影检查的114例患者为研究对象。根据冠状动脉造影结果将研究对象分为冠状动脉粥样硬化性心脏病组(简称冠心病组)(n=86)和对照组(n=28),根据Gensini评分系统将冠心病组患者分为评分<30 分组(n=15)、30~90 分组(n=44)、>90分组(n=27);根据冠状动脉狭窄支数将冠心病组患者分为单支病变组(n=21)、双支病变组(n=25)和多支病变组(n=40)。采用酶联免疫吸附试验检测各组患者血清APN、CTRP9水平,分析血清APN、CTRP9水平与冠状动脉病变严重程度的相关性。结果 冠心病组患者血清APN、CTRP9水平显著低于对照组(P<0.001);对照组、单支病变组、双支病变组、多支病变组患者血清APN水平呈逐渐降低趋势,两两比较差异均有统计学意义(P<0.05,P<0.01);单支病变组、双支病变组及多支病变组患者血清CTRP9水平显著低于对照组(P<0.01);多支病变组患者血清CTRP9水平低于单支病变组及双支病变组(P<0.05,P<0.01);单支病变组和双支病变组患者血清CTRP9水平比较差异无统计学意义(P>0.05)。Gensini 评分<30 分组、30~90 分组患者血清 APN、CTRP9 水平均低于对照组(P<0.05),>90 分组患者血清 APN、CTRP9 水平均低于对照组、<30 分组、30~90分组(P<0.05);血清APN、CTRP9水平与 Gensini 评分呈负相关(r=-0.611、-0.527,P<0.001),APN与CTRP9呈正相关(r=0.384,P<0.001)。结论 血清APN、CTRP9参与冠心病的发生和发展,可作为反映冠状动脉病变严重程度的有效指标。
Abstract:
Objective To explore the correlation of the serum level of adiponectin (APN) and C1q/TNF-related protein 9 (CTRP9) with the severity of coronary artery disease.Methods A total of 114 patients who underwent coronary angiography in the First Affiliated Hospital of Xinxiang Medical University from March to September 2018 were randomly selected as the subjects.According to the angiographic results,they were divided into coronary heart disease(CHD) group (n=86 ) and control group (n=28).According to the score of coronary artery disease with the Gensini scoring system,the patients in the CHD group were divided into <30 scores group (n=15),30~90 scores group (n=44 ) and >90 scores group (n=27).According to the number of coronary stenosis vessels,the patients in the CHD group were divided into single vessel lesion group (n=21),double-vessel lesions group (n=25 ) and multi-vessel lesions group (n=40).Serum APN and CTRP9 levels were detected by enzyme-linked immunosorbent assay.The correlation between the serum APN and CTRP9 level and severity of coronary artery disease was analyzed.Results The levels of APN and CTRP9 in the CHD group were significantly lower than those in the control group(P<0.001).The levels of serum APN in the control group,single-vessel lesion group,double-vessel lesions group and multi-vessel lesions group decreased in turn (P<0.05,P<0.01).The levels of serum CTRP9 in single-vessel group,double-vessel lesions group and multi-vessel group were significantly lower than those in the control group (P < 0.01).The level of CTRP9 in multi-vessel lesions group was lower than that in the single-vessel group and double-vessel lesions group (P<0.05,P<0.01);there was no significant difference in the serum level of CTRP9 of patients between the single-vessel group and double-vessel group(P>0.05).The levels of serum APN and CTRP9 in the <30 scores group and 30~90 scores group were lower than those in the control group (P<0.05).The serum APN and CTRP9 levels in the > 90 scores group were lower than those in the control group,the <30 scores group and the 30~90 scores group (P< 0.05).The serum APN and CTRP9 levels were negatively correlated with Gensini scores(r=-0.611,-0.527; P<0.001),and APN was positively correlated with CTRP9(r=0.384,P<0.001).Conclusion Serum APN and CTRP9 are involved in the occurrence and development of CHD and can be used as effective indicators to reflect the severity of coronary lesions.
