摘要:
目的 评估弥散张量成像(DTI)技术在脑室周围白质软化(PVL)早期诊断中的价值。方法 选择2014年1月至2016年10月河南省人民医院收治的25例PVL患儿作为观察组,另选择年龄与之匹配的10例健康婴幼儿作为对照组。分别对2组受试者进行常规头颅磁共振成像(MRI)及DTI检查,记录相应兴趣部位(ROI)的各向异性分数(FA),比较2组受试者ROI的FA值,并比较观察组患儿康复治疗前后FA值及发育商。结果 与对照组比较,观察组患儿皮质脊髓束(CST)、内囊前肢(ICAL)、内囊后肢(ICPL)、胼胝体膝部(GCC)、胼胝体压部(SCC)、扣带束(CG)等部位DTI图中均存在不同程度的破坏甚至缺失,其相应的FA值显著降低。观察组患儿左侧和右侧ICAL、右侧ICPL、GCC、左侧和右侧CST的FA值均显著低于对照组(P<0.05);左侧ICPL、SCC、左侧和右侧CG的FA值与对照组比较差异无统计学意义(P>0.05)。观察组患儿治疗后右侧ICAL、左侧和右侧ICPL、GCC、SCC、左侧和右侧CST、左侧CG的FA值均显著高于治疗前(P<0.05);左侧ICAL、右侧CG的FA与治疗前比较差异无统计学意义(P>0.05)。观察组患儿治疗后发育商较治疗前明显提高(χ2=8.658,P<0.05)。结论 DTI对早期PVL细微白质纤维损伤患儿具有诊断价值。
Abstract:
Objective To evaluate the value of diffusion tensor imaging (DTI) in the early diagnosis of periventricular leukomacia (PVL).Methods Twenty-five children with PVL admitted to the People′s Hospital of Henan Province from January 2014 to October 2016 were selected as observation group,and 10 healthy children matched with the age of observation group were selected as control group.The subjects in the two groups were examined by conventional head magnetic resonance imaging (MRI) and DTI,and the fractional anisotropy (FA) value of region of interest(ROI) were recorded and compared between the observation group and the control group;the FA values and developmental quotients of children were compared before and after rehabilitation treatment in observation group.Results Compared with the control group,the DTI images of corticospinal tract (CST),anterior limb of internal capsule (ICAL),posterior limb of internal capsule (ICPL),knee of corpus callosum (GCC),pressure of corpus callosum (SCC) and cingulate tract (CG) of children in the observation group showed different degrees of damage or even absence,and the FA values of above area were significantly reduced.The FA values of the left and right ICAL,right ICPL,GCC,left and right CST of children in the observation group were significantly lower than those in the control group (P<0.05);there was no statistic difference in the FA values of the left ICPL,SCC,left and right CG of children between the observation group and control group (P>0.05).The FA values of right ICAL,left and right ICPL,GCC,SCC,left and right CST,left CG of children after treatment were significantly higher than those before treatment in the observation group (P<0.05);there was no significant difference in the FA values of left ICAL and right CG before and after treatment in the observation group (P>0.05).The developmental quotient of the children after treatment was significantly higher than that before treatment in the observation group (χ2=8.658,P<0.05).Conclusion DTI is useful in the early diagnosis of PVL fine white matter fiber injury.
