摘要:
目的 探讨蓝光照射联合茵栀黄颗粒和布拉氏酵母菌散治疗新生儿黄疸的临床效果。方法 选择2017年1月至2018年7月海南医学院第二附属医院收治的144例新生儿黄疸患儿为研究对象,按照治疗方法分为观察组和对照组,每组72例。2组患儿均给予蓝光照射治疗,每日照射2次,每次8 h,连续照射5 d。在蓝光照射治疗基础上,对照组患儿给予布拉氏酵母菌散0.125 g,温水冲服,每日1次,连续治疗5 d。在对照组患儿治疗基础上,观察组患儿给予茵栀黄颗粒1 g,温水冲服,每日3次,连续治疗5 d。记录2组患儿的黄疸消退时间、光疗时间、住院之间,分别于治疗前及治疗后检测2组患儿血清总胆红素水平,分别于治疗前及治疗后使用流式细胞仪检测2组患儿T细胞亚群CD3+、CD4+、CD8+、CD4+/CD8+,于治疗后评估2组患儿临床疗效,治疗过程中观察2组患儿不良反应发生情况。结果 观察组患儿黄疸消退时间、光疗时间及住院时间均显著短于对照组(P<0.05)。观察组和对照组患儿每日血清总胆红素下降值分别为(58.79±17.68)、(34.56±14.25)μmol·L-1,观察组患儿每日血清总胆红素下降值显著大于对照组(t=9.054,P<0.05)。2组患儿治疗前CD3+、CD4+、CD8+及CD4+/CD8+水平比较差异均无统计学意义(P>0.05);2组患儿治疗后CD3+、CD4+及CD4+/CD8+水平高于治疗前(P<0.05),2组患儿治疗后CD8+水平与治疗前比较差异均无统计学意义(P>0.05);治疗后,观察组患儿CD3+、CD4+及CD4+/CD8+水平高于对照组(P<0.05);2组患儿治疗后CD8+水平比较差异无统计学意义(P>0.05)。观察组患儿痊愈51例,有效21例;对照组患儿痊愈27例,有效44例,无效1例;观察组患儿治疗效果优于对照组(Z=4.593,P<0.05)。治疗过程中,观察组和对照组患儿不良反应发生率分别为1.39%(1/72)、5.56%(4/72),2组患儿不良反应发生率比较差异无统计学意义(χ2=0.829,P>0.05)。结论 蓝光照射联合茵栀黄颗粒和布拉氏酵母菌散治疗新生儿黄疸可以快速降低患儿血清总胆红素水平,有效改善黄疸症状,提高患儿免疫功能。
Abstract:
Objective To explore the clinical effect of blue-light irradiation combined with Yinzhihuang granule and saccharomyces boulardii sachets in the treatment of neonatal jaundice.Methods A total of 144 neonatal jaundice children admitted to the Second Affiliated Hospital of Hainan Medical University from January 2017 to July 2018 were selected as the research objects,they were divided into observation group and control group according to the treatment methods,with 72 cases in each group.The children in the two groups were treated with blue-light irradiation,twice a day,8 hours each time for 5 days.On the basis of blue-light irradiation treatment,the children in the control group were treated with 0.125 g saccharomyces boulardii sachets,mixing it with warm water,once a day for 5 days.On the basis of the treatment of children in the control group,the children in the observation group were treated with 1 g Yinzhihuang granule,mixing it with warm water,three times a day for 5 days.The jaundice regression time,phototherapy time and hospitalization time of the children in the two groups were recorded.The level of serum total bilirubin was measured before and after treatment.The T cell subsets CD3+,CD4+,CD8+ and CD4+/CD8+ were detected by flow cytometry before and after treatment respectively.The clinical efficacy was evaluated after treatment,and the adverse reactions were observed during the course of treatment.Results The jaundice regression time,phototherapy time and hospitalization time in the observation group were significantly shorter than those in the control group (P<0.05).The decrease value of serum total bilirubin in the observation group and the control group was (58.79±17.68) and (34.56±14.25) μmol·L-1,respectively.The decrease value of serum total bilirubin in the observation group was significantly higher than that in the control group (t=9.054,P<0.05).There was no significant difference in the levels of CD3+,CD4+,CD8+ and CD4+/CD8+ between the two groups before treatment (P>0.05).The levels of CD3+,CD4+ and CD4+/CD8+ after treatment were higher than those before treatment in the two groups (P<0.05).There was no significant difference in CD8+ level before and after treatment in the two groups (P>0.05).After treatment,the levels of CD3+,CD4+ and CD4+/CD8+ in the observation group were higher than those in the control group (P<0.05),but there was no significant difference in CD8+ level between the two groups (P>0.05).In the observation group,51 cases were cured and 21 cases were effective;in the control group,27 cases were cured,44 cases were effective and 1 case was ineffective;the curative effect of the children in the observation group was better than that in the control group (Z=4.593,P<0.05).In the course of treatment,the incidence of adverse reaction in the observation group and the control group was 1.39% (1/72) and 5.56% (4/72),respectively.There was no significant difference in the incidence of adverse reaction between the two groups (χ2=0.829,P>0.05).Conclusion Blue-light irradiation combined with Yinzhihuang granule and saccharomyces boulardii sachets in the treatment of neonatal jaundice can rapidly reduce the level of serum total bilirubin,effectively improve the symptoms of jaundice and the immune function of children.
