摘要:
目的 了解中枢神经系统感染患儿脑脊液病原菌的构成及耐药情况,为临床医师合理使用抗菌药物提供依据。方法 选择郑州大学附属儿童医院2015年1月1日至2017年12月31日收治的5 681份中枢神经系统感染患儿的脑脊液标本为研究对象,对所有标本进行病原菌鉴定和药物敏感性试验,分析病原菌的分布及耐药情况。结果 5 681份脑脊液标本中,共分离出病原菌146株(2.57%),其中革兰阳性菌119株(81.51%),革兰阴性菌25株(17.12%),真菌2株(1.37%)。位居前5位的病原菌依次是表皮葡萄球菌29株(19.86%)、肺炎链球菌27株(18.49%)、屎肠球菌16株(10.96%)、大肠埃希菌12株(8.22%)、B群无乳链球菌9株(6.16%)。中枢神经系统感染患儿脑脊液标本病原菌检出科室位居前3位的依次是神经内科(31.51%)、神经外科(20.55%)、新生儿诊疗中心(19.86%)。出生0~28 d、出生29 d至1岁、1~5岁、6~13岁患儿脑脊液中位居第1位的病原菌分别为大肠埃希菌(23.08%)、肺炎链球菌(25.45%)、表皮葡萄球菌(29.55%)、表皮葡萄球菌(35.71%)。表皮葡萄球菌对氨苄西林、青霉素的耐药率均为100.00%,对呋喃妥因、替考拉宁、利奈唑胺、万古霉素的敏感率均为100.00%;肺炎链球菌对克林霉素、红霉素的耐药率均为100.00%,对左氧氟沙星、莫西沙星、利福平、万古霉素、利奈唑胺、美罗培南、左旋氧氟沙星的敏感率均为100.00%;大肠埃希菌对氨苄西林、哌拉西林的耐药率均为100.00%,对亚胺培南、哌拉西林他唑巴坦、美罗培南、阿米卡星、氯霉素的敏感率均为100.00%。结论 儿童中枢神经系统感染患儿脑脊液病原菌以革兰阳性菌为主。临床医师应根据菌株特点和药物敏感性试验结果,合理选用抗菌药物。
Abstract:
Objective To understand the composition and drug resistance of cerebrospinal fluid pathogens in children with central nervous system infection,and to provide basis for clinicians to use antibiotics reasonably.Methods From January 1,2015 to December 31,2017,5 681 cerebrospinal fluid samples from children with central nervous system infection were selected in the affiliated Children′s Hospital of Zhengzhou University.Pathogenic bacteria identification and drug sensitivity test were carried out on all specimens,and the distribution and drug resistance of pathogenic bacteria were analyzed.Results A total of 146 strains (2.57%) were isolated from 5 681 cerebrospinal fluid specimens.Among them,119 strains (81.51%) were Gram-positive bacteria,25 strains (17.12%) were Gram-negative bacteria and 2 strains (1.37%) were fungi.The top five strains were Staphylococcus epidermidis (19.86%),Streptococcus pneumoniae (18.49%),Enterococcus faecium (10.96%),Escherichia coli (8.22%),Group B Streptococcus lactis(6.16%).The top three department of pathogens in cerebrospinal fluid of children with nervous system infection were neurology (31.51%),neurosurgery (20.55%) and neonatal diagnosis and treatment center (19.86%).The first place of the pathogens in the cerebrospinal fluid from 0 to 28 days after birth,29 days to 1 year old,1 to 5 years old,and 6 to 13 years old was Escherichia coli(23.08%),Streptococcus pneumoniae(25.45%),Staphylococcus epidermidis(29.55%),Staphylococcus epidermidis(35.71%).The drug resistance rate of Staphylococcus epidermidis to ampicillin and penicillin was 100.00%,and the sensitivity rate to furantoin,tetrandrine,linezolid and vancomycin was 100.00%.The drug resistance rate of Streptococcus pneumoniae to clindamycin,erythromycin was 100.00%,and the sensitivity rate to levofloxacin,moxifloxacin,rifampicin,vancomycin,linezolid,meropenem and levofloxacin was 100.00%.The drug resistance rate of Escherichia coli to ampicillin and piperacillin was 100.00%,and the sensitivity rate to imipenem,piperacillin,tazobactam,meropenem,amikacin and chloramphenicol was 100%.Conclusion Gram-positive bacteria was the main bacteria in cerebrospinal fluid of children with central nervous system infection.Clinicians should choose antibiotics reasonably according to the characteristics of strains and the results of drug sensitivity test.
